You have a prominent and relatively heavy nipples that are no longer supported by firm breast tissue. You either have to replace the volume or tighten the areolar skin. One way to address that and also provide volume to both breasts if fat grafting. Alternatively, small implants with just subareolar fat graft would give a predictable breast volume without extensive fat harvest and potential issues with donor sites.

You lost some volume in your breasts, so replacing the volume will help fill out the breast under the areolar area. This can be done with implants or fat. Your nipple size is fine and does not need to be larger.
