Piercing migration scar: I had my nipples pierced for about 5 years and just took them out due to a migration issue. The right one migrated down and the nipple became enlarged around where the piercing currently sat. Now it looks like I have one smaller nipple below the original piercing site. I’m wondering if it will stay this way or if it needs surgical correction.

The piercings would have to be removed for surgery. Once the nipples were repaired and healed, then the piercing could be reinserted. Unfortunately, due to the piercing, you are at higher risk of infection from the bacteria that lives in the tunnel.
Yes, the lower areola can be raised to better match the opposite areola, or the higher might be lowered some. Please go back to your surgeon and discuss your concerns. Your nipples looked fine.
