SculpSure works because surface cooling keeps the deeper dermis from being injured and the 3cm depth of penetration typically would only hit fat. On skin with less than 2-3cm of fat (like a thin neck), for safety, we do not use it. Also, If SculpSure didn't have built-in cooling cycles, the heat could slowly spread beyond 3cm and could damage underlying tissues. Fortunately, between the built-in cooling cycles and us asking patients about their discomfort, I don't believe injuries to organs have ever been reported :)
