Without seeing your pre-surgery photos, it's difficult to determine what has occurred here. The surgeon may not have removed enough tissue, but there are other reasons why breast tissue can return. These can include weight gain, hormone replacement, pituitary issues, cannabis use, etc. I recommend you consult a board certified plastic surgeon to discuss options to improve the contour of your chest.
