6 months post-op - what does it mean if nipples now look like they did before? (Photos)

I had gynecomastia surgey six months ago. Roughly a month and half after the surgey, my nipples started swelling again, I thought it was seroma and it will go away after while, but things were getting worse, I went to my doctor, he said it is blood clot and I need to wait another six months, which didn't make sense to me. I searched online and couldn't find any case like mine. Is this a result of failed surgery? Did my surgeon mistreat it by removing fat tissue and not breast tissue?
