I'm torn between sizes and which type of implant. I am a 32-34 AA in bra size currently, my goal is a full C cup, small D cup. I have broad shoulders and my chest is a little wider and I want to balance my body out. I want them to look natural but also have cleavage. I am choosing to go under the muscle. I have heard that going under the muscle you lose volume of the implant, so I'm concerned if I go with 275cc I will not be completely satisfied with my size.

Surgeons have different techniques for determining how to help patients choose their implant size. Placing implants under the muscle does make them appear slightly smaller. If patients have an implant that they are happy with when trying them on with a sports bra, I recommend going up one additional size. This is because the muscle puts pressure on the implant and makes it appear slightly less full.

Answer: Breast Implant Sizing The choice of size and profile is a very challenging one that we spend the most time on in our consultations. We employ 3D imaging and simulations, rice sizer's, the Mentor gel sizing system, as well as several biodimensional measurements taken on the breasts themselves to come up with the best plan for our patients. The good news is you seem to have narrowed down your choices to a small range. Your breasts may increase or decrease in size by 20 to 30 mL over the course of normal weight loss, weight gain, monthly changes in your breasts with your hormonal cycle, etc... so if 275cc is a good size, 300cc is likely going to give you a good result as well. Most patients will typically wish they had gone a bit bigger when the swelling is done subsiding, so our patient will tend to choose the larger of the two sizes when they are stuck deciding between two similar implants as you are.

Generally speaking, the best online advice I can give to ladies who are considering breast augmentation surgery ( regarding breast implant type/size/profile selection) is: 1. Concentrate on choosing your plastic surgeon carefully. Concentrate on appropriate training, certification, and the ability of the plastic surgeon to achieve the results you are looking for. Ask to see lots of examples of his/her work. 2. Again, have a full discussion and communication regarding your desired goals with your plastic surgeon. This communication will be critical in determining which operation and/or breast implant size/type/profile will most likely help achieve your goals. In my practice, the use of photographs of “goal” pictures (and breasts that are too big or too small) is very helpful. I have found that the use of words such as “natural” or "C or D cup" etc means different things to different people and therefore prove unhelpful. Also, as you know, cup size varies depending on who makes the bra; therefore, discussing desired cup size may also be inaccurate. Therefore, I ask that my patients not base their communication preoperatively OR their satisfaction with the outcome of surgery postoperatively, on achieving a specific cup size. The use of computer imaging may be very helpful during this communication phase. 3. Once you feel you have communicated your goals clearly, allow your plastic surgeon to use his/her years of experience/judgment to choose the breast implant size/profile that will best meet your goals. Again, in my practice, this decision is usually made during surgery. The use of temporary intraoperative sizers, viewing the patient's breasts in the upright and supine position, are very helpful during the breast implant selection process.
