Lumpy filler under eye, can it be fixed? The new cannula injections seem to be a mess for me.

Generally speaking, there is a trend to always use cannulas in order to avoid bruising. This is reasonable, especially when injecting into deeper areas of the face. However, cannulas decrease accuracy in very fine areas like tear troughs. I generally use injections in this area as I would emphasise accuracy over bruising which patients appreciate in the long-term. 

Fillers can often absorb water in the first few days causing lumps or irregularities. I suggest applying Plato's Bruise Balm twice daily for a few days after injecting my patients. If the filler lumps do not resolve, then a small injection of Hylenex usually can correct an HA filler that is overdone. 
In some cases, Venus Viva RF can help tighten skin thereafter.

If you're unhappy with your final results, you can have it reversed with hyaluronidase and try a different technique next time. Keep in mind, a combination approach will give the most improvement. Fillers, lasers, and microneedling/PRP can improve under the eyes. My patients try to get non surgical treatments to avoid surgery. We use PRP and stem cells there often. I suggest consulting with an expert to develop a treatment plan best suited for your short and long term goals. Best, Dr. Emer

Thank you for sharing your question and I am sorry that your cannula results have not been what you anticipated.  Under eye fillers can still be performed with needle insertion if you prefer in the hopes of avoiding lumps but there is a much greater risk of bruising.  Cannula use does take practice so be sure to vet your injector. 

In my hands, cannula injections reduce the risk of lumpiness.  There is also a much lower risk of bruising with cannulas.  If you have lumpiness, it is likely an injector/technique issue, not an inherent problem with cannulas.Regards,Dr. Ort

Not all cosmetic injectors or filler products are equal. I would wonder if your experience has more to do with the injector, or the product being injected, instead of the cannula. Every injector has a preference in cannula verses a needle in the technique they use. I personally use a cannula, and have never had problems with a "bumpy" look in my patients. If you really want your tear troughs done using a needle technique. I recommend calling different Board Certified Dermatologists or Board Certified Plastic Surgeons in your area and asking if they are using a needle verses a cannula to inject under the eyes.

Cannula's are NOT required for under eyes, some doctors like them because they reduce the chance of bruising, but it can lead to lumpiness, in our office we do both straight needle and cannulas depending on the look and needs of a client!
