i’m not interested in getting a brand new nose i just want my nose but better. i’m considering alarplasty to simply reduce the width but i’m wondering if maybe i could use a full rhino to also slim the bridge of my nose to ensure a proper symmetry. do you think my nose would be fine with just a alarplasty or would i need further work to get this desired effect? (i used a editing app to adjust only the width of my nostrils on the second picture)
Answer: Closed and Atraumatic Technique in Rhinoplasty Hello, Thank you for your question. Since a nose is a 3 dimensional structure while reshaping the nose, all nasal structures and whole face proportions should be taken into consideration. Therefore when alar base reduction is done the tip should be harmonized with the dorsum and the airway deterioration should be avoided. I have developed a Novel Technique in Rhinoplasty called as Superior-Based Transposition Flap (ST) to reshape, relocalize and reinforce the LLC as a support material for both internal and external valves with aesthetic corrections, leaving the scroll area untouched. I use suturing techniques to achieve a narrower, refined and aesthetically pleasing tip without the use of any grafting technique. Comparing to the open rhinoplasty Closed and Atraumatic technique in Rhinoplasty minimizes swelling and bruises; maximizes healing, prevents tissue damage and eliminates a scar on the columella. Answer: Rhinoplasty Thank you for including photos with your question. In the morphed image you attached, you are slimming the bridge as well as the tip of the nose and narrowing the nostrils. These changes can only be made with a rhinoplasty and alarplasty. I would suggest an in-person consultation with a rhinoplasty specialist. During a consultation a computer simulation can draw potential changes to your nose. That will give you a very accurate idea of what you could anticipate with rhinoplasty and alarplasty.

Answer: Alarplasty or would i need further work ? Improving the alar width is a frequent request of patients wanting nasal refinement. It is possible to improve the cosmetic appearance of the tip alone without altering the rest of the nose in some patients. Whether the optimal option in your particular situation would be a tip rhinoplasty, alar-plasty alone, or an artistically performed natural appearing full rhinoplasty would best be determined at a consultation. Both a tip-plasty or a full rhinoplasty procedure can be performed with a closed or open approach. Following the advice from a surgeon on this or any other website who proposes to tell you exactly what to do based on limited 2 dimensional photos without examining you, physically feeling the tissue, assessing your desired outcome, taking a full medical history, and discussing the pros and cons of the operative procedure would not be in your best interest. I would suggest you find a plastic surgeon with extensive nasal surgery experience who is certified by the American Board of Plastic Surgery and ideally a member of the American Society for Aesthetic Plastic Surgery (ASAPS) or a facial plastic surgeon (otolaryngologist) that you trust and are comfortable with. You should discuss your concerns with that surgeon in person. Robert Singer, MD FACS La Jolla, California

Answer: Alar- plasty candidate An alar-plasty only narrows wide nostrils, and that's it. A tip rhinoplasty is required to narrow the nasal tip cartilages. A full rhinoplasty is required to narrow the entire nose.
