2 weeks post-op I still need an alar base reduction. What are my options and how soon can I have another procedure? (photos)

I had a rhinoplasty two weeks ago. One of my primary goals is to reduce the alar base but my surgeon didn’t perform it during the procedure as he’s planned before the surgery. Now after the surgery I’m still quite unsatisfied about the width of the nose and nostrils. Except this I’m ok with other outcomes. I tried to contact my surgeon but he gave no response. I’m considering a small surgery just to reduce the width of the alar base and nostrils and wonder when could it be done.

Hello, Thank you for your question. Certainly it is so early to evaluate your rhinoplasty results because of the healing process. At least one year we need to wait to see how it improves from current state. I can understand your concerns and worries about this issue. Alar base reduction can be performed under local anesthesia in one year following the operation. If I am in any doubt, I quite regularly refrain from doing alar base reductions, just in case I am wrong. It can be done easily under local anaesthetic at any time but I would generally like to wait for three months in order for the tissue to settle down but also to make sure it is the right thing to do. There is no going back once you've done it!

Usually most surgeons would like to wait a minimum of 3 months after surgery to allow tissues to settle. Be aware that an alar base reduction will narrow your nostrils and change their shape from round to elongated. Find out why he decided not to do an Alar base reduction if that is what you expected and had discussed prior to surgery.If you have any concerns return to your PS for advice and guidance.

I am glad to see your post-rhinoplasty 2 weeks result.A good job has been done.Alar base reduction can be done any time you plan to do after first 3 months of this surgery.This is done from inside and does not leave any external scars.
  
If it was a open rhinoplasty than alar base reduction cannot be combined for the safety of healing of the skin over the tip, and this may be reason your PS did not do it.A superficial to deep skin necrosis at the tip of the nose is a possibility if the blood supply from the alar base is cut by cutting the alar base.
