I am 45 years old and have had fillers to increase volume in the mid face and improve the under eye 4years ago. I’d like to do something that will last longer and help with eye hollowness, and dark circles, loss of mid face volume, and mid face sagging. Surgery seems to be the best option at this point. My question is what procedures would work best for my anatomy and concerns. Thanks!
Answer: Blepharoplasty and midface lift Great questions! Surgery is absolutely the longer lasting option. You would of course need an examination to determine lid laxity, vector analysis, among other things..but from the pictures alone I think you would be a great candidate for a lower blepharoplasty with midface lift and volume augmentation via fat grafting. By repositioning the fat in your lower lid, not just removing it, we can eliminate the dark circles and smooth out the nasojugal groove as well. Finally, for the optimum results a concomitant profractional laser will tighten up that periorbital skin. It looks like you may have a bit of upper lid ptosis (droop), that if addressed would also give a more youthful and energetic appearance, but that is often just related to the photograph/camera technique and again would need to be examined!

I recommend a frontal browlift to lower your hairline, decrease the vertical height of your forehead and pull up the eyebrows. Answer: I strongly recommend removing all of your facial filler. The filler is old enough that it is holding extra water and making you look puffy. That is relevant because that extra volume actually makes you look 10 years older than you should. Remove the filler and instantly you will look much younger. No the enzyme does not "wreck" your skin. You may need a very small amount of filler in the under eye area and around the mouth. You do have upper eyelid ptosis and I would recommend having that thoughtfully corrected.
