Did my surgeon over dissect my pockets? I have started wearing a thong bra 24/7. (photos)

Thanks for your question and for including photos. The inner edge of the implants are closer together, but the photos don't look like you have synmastia or uniboob. Part of the reason is the change in implant size. I'd wait for total of 6 months after surgery before thinking of a revision. Thong bra seems like a reasonable idea in the meantime.

The distance between the two implants is quite small. This makes the breast look a natural. For the large volume you chose a different profile with a smaller diameter implant might be a better choice to prevent dissecting the pocket to receive the implant beyond the normal anatomic borders of the breast.

From your photos, it would appear that the inner part of the breast pockets have been disrupted , creating a near symmastia. This was not present in your preoperative photo. This can be corrected with revisional surgery. I would recommend consulting an experienced board certified plastic surgeon who performs frequent revisional surgery to discuss your options. The problem will not resolve on its own.

 Without an exam it is difficult but I have a tendency to agree with Dr. Sheridan. I think the majority of the misshapened cleavage could be attributed to simply an almost 600 mL implant. When going much larger a previous pocket has to be opened up to allow for the size or the implant has a tendency to bulge or fold and that kind of looks like what is happening with you.

Thank you for sharing your excellent question and photographs.  Unfortunately without an in-person examination it is difficult to offer definite advice.  Your pictures indicate a clear demarcation between your implants and as a result I do not think that you have synmastia.  It is not uncommon that as the implants drop and fluff, you will see a narrowing of the gap between the implants over the breast bone but once fully healed this gap widens again.  I would recommend giving your results more time to mature, but if the thong bra provides reassurance, go ahead and keep using it.  Hope this helps.

Hi Futureboobies.I am sorry that you were deceived by the cosmetic surgeon.  Obviously, once trust is violated, it mars credibility.That out of the way, I would say that you need to wait things out.  The good news is that you have cleavage, so there isn't "synmastia", or one breast.  On the other hand, the cleavage is a bit more crowded than before.  This isn't a bad thing, as there isn't a standard distance that the cleavage should be.You need to be patient and keep a lid on the anxiety, as the healing process may take a year.  I think that chances are that the aesthetics will improve over time.  So far, so good.sek

Dear futureboobies,Thank yo for your question.  Ther are I feel 2 questions here. 1st was ther over dissection of the pocket? 2nd are your new implants too large for your tissues?It is of course impossible to know if an over dissection was done, but by the looks of your post-op photos I do not think so. However, the large implants are obviously stretching the tissue causing your brest to expad and come together near the midline.  The thong bra will help this.  If your not satisfied with the appearance at 6-8 mos you may want to consider changien to a smaler sized implant.  Hope this helps & good luck.James Sheridna, MD

I am sorry to hear about your concerns after revisionary breast surgery.  Although symmastia is not present,  I do understand your concerns about the breast implants being quite close to one another in the midline. 
If implant positioning remains a concern longer term, I think that you will benefit from bilateral medial breast capsulorraphy (internal suture “repair”) as well as lateral capsulotomy to improve your situation.  
In my practice, having used a variety of “techniques” for correction of medial implant malposition, I find that the most reliable technique involves a 2 layer capsulorrhaphy (internal suture repair of the breast implant pockets along the cleavage area). Often, the use of acellular dermal matrix is helpful also. I have also been pleased with the (at least partial) correction of skin tenting that can be achieved with the use of the acellular dermal matrix.
 It is often necessary to “open” (reverse capsulotomy) the breast implant pocket laterally (outer breast fold) to allow for positioning of the implant centrally behind the breast mound. This maneuver may also decrease the amount of implant pressure against the medial suture line. 
It may also be necessary to change breast implant profile or volume to help prevent chances of recurrence of breast implant malposition.
This procedure will serve to improve the position of your breast implants on your chest wall, better center the nipple/areola complexes on each breast mound.
I also find that careful attention to postoperative activity restrictions is key to success with this type of surgery and other types of implant malposition revisionary breast surgery.
