Gynecomastia revision - can this be fixed with a periareolar incision? (Photo)

Question

I had a Gynecomastia surgery 9 months ago. As you can see from the pictures, the results are uneven with over resection on the right side and redundant skin and some gland/scar tissue leftovers on the other side. I also feel like the areola is lower than where it’s supposed to be. Is this something that can be fixed with a periareolar incision approach or will fat grafting be necessary. I also hear that fat grafting doesn’t have a good reputation as it doesn’t work long term.

Improving over resection after gynecomastia surgery is simply difficult, and there are no easy answers.
In your case, part of the problem is that you have significant skin redundancy. A proper chest lift could bring soft tissue and improve the contour, but add significant scars.
Sometimes it’s better to cut bait because revision surgery isn’t justified based on the amount of improvement that you’ll have. It’s a balancing act and a lot depends what you’re willing to go through and of course finding the right provider.
