How can I create slightly more fullness in the upper pole, while keeping the lower pole shape I like? (Photos)

I had breast aug. a year ago with anatomical Mentor CPG333, 380cc implants using the dual-plane with internal bra. My natural breasts were slightly tubular, nipples pointed down.I’ve lost almost 50 kg before,so skin is less tight.The main reason for considering re-augmentation is a deformity at the border between the gland and implant in my right breast,visible when flexing muscles, moving my arm,or bending forward. I do not want smaller breasts. I like the overall shape, especially the lower pole and side boob projection, but the deformity bothers me and I would like slightly push up effect in the upper pole and bigger breasts. During a recent consultation, the surgeon said the gland is bilaterally freely movable without a waterfall effect. He suggested: 1:Only fat transfer to cover deformity. 2:dual-plane aug., adjustment of the implant capsule, reconstruction of the inframammary fold, smoother round implants to prevent rotation, fat grafting,GalaFLEX,shortening the gland.Motiva Round 590cc Ultra High Profile were suggested, which I initially agreed to, but I fear they could create a ‘pornstar effect,’ overly large breasts, undesired upper-pole projection, or downward-pointing nipples. I want a balanced result with slightly more fullness in the upper pole, while keeping the lower pole shape I like. Which implant type, brand, size, and texture would you recommend to achieve this? Would using an anatomical implant again be really in this case unsuitable for possibe rotation?
