Am I a candidate for this type of rhinoplasty ? (Photo)

Hello Straxville,It would benefit you to find a surgeon who is both a Board-Certified Plastic Surgeon and a Board-Certified ENT.  These are the unique qualifications that bring many patients to my door.  A chin implant combined with reduction of your columellar show would help you achieve the more balanced look you desire.  In addition you might consider a small graft to your nasion region (the point at which your nose meets your forehead) which would reduce the appearance of your nasal projection in profile view.Best Regards,Lane Smith, MD, FACS

of rhinoplasties and rhinoplasty revisions each year. Then look at the
plastic surgeon's website before and after photo galleries to get a

Rhinoplasty for de-projection of the nasal tip and chin implant.  You will need an extension septal graft and possible graphs to both alas in order to lengthen the nose. I would need to examine this to give you a proper answer. See a very experienced revision rhinoplasty surgeon for this type of expertise.

What you are referring to is a de-rotation of the nasal tip. This will require multiple cartilage grafts to drive the tip downward and lower the alar rims. It is not as easy to perform as some rhinoplasties but that does not necessarily make it unduly difficult or unwise to do. You would benefit by a chin augmentation whose amount of forward projection is best determined by your response to various computer imaging options. 

 The photo displays over projecting nasal tip and significant columellar show with a high nasolabial angle.  Reducing the nasal spine,  reducing the columellar show(Collumellar-plasty) by trimming back skin and cartilage can help with the  downward rotation of the nose and help with the bottom heavy look and nostril show. The goal would be to de-project and de-rotate the nose. Placing a small chin implant through a sub mental approach will give more projection to the chin and give the illusion that the nose has been de-projected. Chin implants are manufactured in a variety of sizes and shapes, are composed of Silastic and can be inserted under either local or general anesthesia.

Thank you for the photos and I would find a surgeon in or out of state who will take your photos digitally and with your input alter them digitally until you see the result you desire.  Then assuming that result is realistic, ask your surgeon how it can be accomplished so you are fully informed.Dr Corbin

Extension cartilage grafts will be necessary to lengthen your nose. Rotation and projection can be decreased at the same time. These are challenging techniques but a natural result will be obtained if you have the work done by a rhinoplasty specialist.

A chin implant would definitely bring a bit more balance to your face and lessen your rhinoplasty needs. You need a bit of deprojection or your tip and correction of the columellar show. This would likely result in the improvement you are looking for. This is not necessarily a very difficult operation, but you do have to find a surgeon with a wide expertise in rhinoplasty.
