Hello,

I would strongly discourage you from injecting any filler over an implant in your nose. Implants have the risk of getting infected at any point, even decades after they have been inserted. Injecting any substance such as filler that are extremely safe in the majority of cases could introduce bacteria that could lead to an infection of the implant and require its removal. What filler lasts the longest in the mandibular angle and jawline?

There are several fillers that can be used in this area that are long lasting. My preference would be using Restylane Lyft since it provides very good definition and lasts 12 to 18 months. I would not use anything that is permanent or semipermanent that could not be easily removed or dissolved. I hope this helps.

Is it true that a low dose of Accutane helps with rhinoplasty healing for thicker skin?

Yes. Good outcomes have been obtained by surgeons in different parts of the world and inmy own experience with low dose Accutane treatment started about three weeks preoperatively and continued for about three months postoperatively. It helps "thin" the skin and allows for better and quicker definition of the nasal tissues.

Will I benefit from injecting 1 syringe of filler a month for tear troughs and cheeks instead of doing it all at one time?

It is fine to do fillers spaced out a month apart or so. You will end up with the same result in the end. I would suggest that you have the cheeks (mid face) done first though since this will improve the area overall and will affect how much filler you need in the tear troughs. Good luck.

It is hard to assess your facial proportions with one photograph only. From what I can see, I don't think you need any anterior projection to your chin. It may be that a vertical or lengthening type of implant may be of benefit, but without seeing a front and three quarters view of your face it is just a guess.

You are definitely a candidate for both procedures. I think that a sliding genioplasty (after a proper physical examination and mandibular x rays are performed) would be your best option to give you better balance to your face. Best of luck.

Mandibular / mental nerve swelling or irreversible damage post chin implant? (Photo)

I am sorry that you have had this problem. Chin implant surgery is usually very simple and complications are rare, but can happen. From the photos you posted it appears that your marginal mandibular nerve is affected (your mental nerve provides sensation to part of your lower lip). This could possibly be due to the excessive swelling that you had. At this point it is better to just wait for several months more until things setle down and the nerve function returns (it likely will). As far as the sensation of the implant when talking, that should be temporary as well. You should continue to follow up with your surgeon and follow his or her advice. If there is a need to replace or reposition the implant your surgeon should be able to take care of that.

Does bedside manner and trust factor in the surgical results?

This is a very important question. In reality, the surgeon's bedside manner would not impact on his ability to provide you with an excellent outcome. The problem is the trust factor, your level of comfort and the ongoing relationship with the surgeon and his staff should there be any issues during your recovery or if a "touch up" is needed. I strongly feel that both a patient and a surgeon have to "click" and be comfortable with each other.

9 months post rhinoplasty, what are my options? (Photo)

You have been given good advice. You do need to wait at least a year before deciding on any further intervention. It may be easy enough to just rasp down the remaining bump and/or use some steroid or 5FU injections to help refine the soft tissues of the tip. Listen to your surgeon and work with him or her to try to imporve your outcome.

A Stranger Complimented my Nose!

I had my 7 months post-op today and felt like I should repost here too. It was this time last year that I was working up the nerve to consider a revision rhinoplasty, and that’s when I came across Dr. Gantous. He is an excellent surgeon and an artist, as well as one of the few in Canada who will even touch noses that have had prior surgery, after I had a bad experience with an ‘instagram famous’ California surgeon. For some background, I wasn’t the typical “bump on the nose” patient that many surgeons see. I was pretty happy with my side profile prior to my first surgery, but my issue was my front view. I had a drooping bulbous tip and a non-harmonious nasal shape (from my research, front-view cases often require a more experienced surgeon). Once I hit my late teens, I felt like my nose suddenly looked mismatched from the rest of my facial features which I liked. I would feel okay in person, but then very few of my pictures would turn out nice because my nose was so distracting. After I built up the courage to get surgery, I sought out a surgeon who was said to be specialized in drooping nasal tips...Unfortunately my first rhinoplasty left me with a wide nose, which by 4 months post-op was also starting to droop again. Ironically even my profile was negatively affected! I was very anxious about having to go through the process again, but Dr. Gantous and his staff were reassuring and made me feel comfortable. He was very thorough in explaining what he would do and used imaging to create projected-after-photos. He knew exactly how to incorporate my desired changes while making sure my nose fit my features, my ethnicity, and looked natural (I’ve kept my rhinoplasty mostly a secret). He used a rib cartilage graft to support the nasal tip and the discreet rib incision scar has been healing nicely too, so do not worry if you require one. Now, not only is my front view harmonious like I always wanted, but my profile is even nicer than it was originally! I love how elegant it looks with its subtle supratip break. If you are looking for a rhinoplasty or a revision, I would highly encourage you to contact Dr. Gantous’s office. I am so thankful I did!

I'm not one to leave reviews, but I really felt that this one is well warranted! I had started looking into getting a septo-rhinoplasty last year after having had a septoplasty which wasn't very successful years ago. Since I already had a surgery on my nose, many surgeons in Toronto would not perform on me, which definitely added to the pressure of choosing the best surgeon from my limited options. I did a lot of research on a few surgeons who were willing to perform on a nose that has already been "touched," including Dr. Gantous. After reading so many reviews on different surgeons and getting overwhelmed, I told myself to just go ahead with the consultations and see who clicks. Upon meeting Dr. Gantous, I felt that he was very humble, kind, professional and genuine. I really wanted to have a surgeon who took the consultation and process seriously, which I did not feel in some of my other meetings. This is a major decision and I didn't want it to be taken lightly by my surgeon. After finishing up all of my consultations, I decided that Dr. Gantous was the best person to perform on me. I was anxious getting closer and closer to my surgery, but Dr. Gantous did assure me that he'll give me the results that I desire. I really wanted a natural look, but of course an improvement on my nose's appearance and breathing. Surprisingly, the day of the surgery was the calmest I had felt during this entire process. I was excited to finally get rid of my biggest insecurity. The surgery went well and afterwards in the recovery room, Dr. Gantous stopped by to let me know that it was successful and my nose looks pretty which was very reassuring. The swelling process is probably the most annoying as you're so anxiously waiting to see how your nose will finally end up, but week by week its been getting smaller and looking better and better. It's now 3.5 months after surgery and I'm loving my results! I feel so much more confident and the most common feedback I'm receiving is that my nose looks completely natural and my eyes stand out a lot more! I can also breathe better which is amazing. I really love the results and I feel like Dr. Gantous did a fantastic job. I would recommend Dr. Gantous to anyone looking for a rhinoplasty and have already given his information to a couple of my friends!

Great revision rhinoplasty results with Dr. Gantous!

I had a revision rhinoplasty with Dr. Gantous in October 2022, after a primary surgery elsewhere. Using an ear graft, Dr. Gantous rebuilt the bridge of my nose and fixed my breathing. He also refined my boxy tip and it looks very cute now. Consultation – Dr. Gantous examined my nose thoroughly and answered all my questions (the list was long!). He also explained the procedure in detail (as a revision rhinoplasty patient you actually do want to hear the details). He finally assured me he was able to fix my nose and he did exactly what he promised.Day of surgery – I was a nervous wreck. Dr. Gantous came to talk to me right before I was taken to the OR. He once again went over the details with me and displayed an aura of confidence and calm which put me to ease. Shortly after the surgery, when I was in the recovery room, Dr. Gantous came to me again, said that he achieved what he aimed for and added that he was happy with the outcome. It meant a lot to hear those words in that moment.Recovery – I was pleasantly surprised how smooth the recovery was. There was almost no bruising although that’s probably because no bone work was done this time around but even the swelling was minimal. There was some discomfort and pain during the first three days however completely manageable with painkillers. A couple days after my cast was taken off, I felt confident to go out in the public – I looked almost completely normal with swelling so minimal that probably only I could notice.I would like to recommend Dr. Gantous and say that you are in the best hands possible with this doctor. Once you meet him in person, you will know what I’m talking about. Apart from his knowledge and expertise, Dr. Gantous is also a caring and ethical doctor who takes his time to listen to the patient while working out the best plan of action. Dr. Gantous gave me my self-confidence and peace of mind back. When I was sitting in his waiting room, I overheard another patient saying that Dr. Gantous is a doctor from God and I couldn’t agree more. Thank you, Dr. Gantous for everything you did for me!Last but not least - I would like to mention that Dr. Gantous works with a team of wonderful and dedicated people. Vanessa, the office manager, made sure everything went smooth. She communicated with me and answered all my questions in detail and in a timely manner. And Joanne, who is the head nurse, ensured I had all the pre-op and post-op information and even took her time on a weekend to answer my follow up questions through a text message.

Cannot describe how happy I am - I can breathe properly

I cannot describe how happy I am since I got my septoplasty with Dr. Gantous! It might be a little early since I am not yet 3 months post-op, however I cannot contain it. I can breathe properly! For months on end I had trouble sleeping because I could only breathe from one side of my nose, especially after getting pregnant with my son- somehow seemed to make the problem worse. I am honestly glad I did. Not only did Dr. Gantous help with my breathing, it also helped with my confidence since I hated my nose growing up too- the family trademark was simply too big for my face. My dorsal hump is a hump no more, my septum does not look like it’s pointing to the east coast, and my nose tip does not droop over my upper lip when I smile. It’s a great feeling! Everyone at the TFP has been amazing from the start. Vanessa was very prompt to reach out to me when I requested a consultation through real self. She explained all the steps I need to take in detail very well. She has been extremely professional and to the point. The nurses that have worked with me were also great, though unfortunately their names escape me. When I received my preop call they had been very patient with explaining the things I needed to do, what I can and cannot do before and after the surgery. I also have to thank everyone at the office including Dr. Alomani who answered my frantic call after fainting the next day because I moved faster than I should have- probably causing my blood pressure to drop. Overall, I cannot wait to see how this journey continues. I know i have a bit of healing still to go, but I am glad to have made my decision to come here.

Talented, Very Skilled, and Compassionate; a Winning Trifecta!

From the moment I walked into the office and was greeted by Vanessa I felt relaxed. Each person I dealt with gave me a sense of competence and comfort. Each member of the clinic team was clearly selected for the task they were doing and carried it out as if you were the only person they had to deal with in that day. Compassionate, caring, and competent. Dr Gantous is first and foremost a talented surgeon who clearly possesses a passion for the work he does. Better yet, he has the skills to back up his surgical offerings and provides outcomes that meet (or exceed) expectations. I came to Dr Gantous with a botched and complicated nose surgery history, including a final emergency surgery, which left me with a saddle nose defect. This sort of revision surgery is complicated due to the amount of “fiddling” that has been done previously and the surgeon not knowing precisely what to expect until you are in surgery. There were plenty of doctors who were willing to “give it a try” but I learned as I went through my list of top-pick doctors, that not every surgeon who is willing to perform a surgery is an expert in the particular surgery being offered. Dr Gantous is an expert in saddle nose deformity with rib graft and more. Upon meeting Dr Gantous, it was clear that his superior skill set is paired with a warm, caring personality. This meant that I was in good hands, and I was never disappointed. Dr Gantous took all the time necessary to understand my concerns and hopes. He was very patient in looking at images with me and explaining the procedure clearly. As well as what was possible given my situation. He never overpromised but was always reassuring about how close to my old face this surgery would take me. He was spot on in his suggestion of what he could do, and the final outcome actually exceeded my expectations. In fact, for the first time in two years my mobile phone’s facial recognition unlock function recognized my face. That tells the story right there! Before my surgery, Joanne, the clinic nurse, gave me a full explanation of the surgical and clinic procedures and what I could expect in terms of after care. Both Vanessa, the clinic receptionist and Joanne were very thorough about what I should do in the lead up to the procedure and this was extremely helpful and reassuring. Despite the surgery taking place during covid times I completed each one of the steps and was in and out of the clinic without any complications. They also provided very detailed instructions for my after-care person and best of all, it all went off without a hitch. I have been well cared for from the moment I entered Dr Gantous office. I would highly recommend Dr Gantous and his team for surgery. Each on of us wants to look as good and as “normal” as possible, and this clinic can make that happen. I would trust Dr Gantous if I ever needed or wanted another surgery.

Asian rhinoplasty & tip plasty revision - perfectly symmetrical to my face

I am 3 weeks out of my revision rhinoplasty surgery and I am so happy, over the moon with my result that I want to share my experience. First, I want to stress how difficult it was to even find a plastic surgeon that does revision rhinoplasty in the Toronto area. I had located a few on Google that claimed they did revision rhinoplasty on their website but once I had contacted their offices, they either did not provide that type of surgery and some no longer do it. I did not realize, how difficult it would be to find a revision rhinoplasty surgeon until then. I had come across a patient on realself who had a revision done in Toronto but when I contacted this doctor, they referred me to Dr Gantous as they were not taking on anymore revision rhinoplasty patients. Dr Gantous' office scheduled my appointment right away. Their reception does a great job answering phone calls and responding to emails in a timely manner. A brief history about me is that as an asian patient, back in the days before Google reviews (early 2000's), we were told to go to a board certified plastic surgeon that specialized in our ethnicity and we should get a good result. So I went to the prominent Asian plastic surgeon at the time, which left me with unsatisfactory results. The teflon bridge graft was on the right side of my nose bridge, the cartilage tip graft was sitting on the right side of my nose tip leaving me with a wider nose that leaned to the right with little to no visible upward projection and the open scar on the columella was visible and prominent. Hard to believe a nose job could leave you with a wider, flatter nose, but that's what happened to me. Being that this was one of the only Asian surgeons in Ontario at the time, I then went to South Korea to have his work corrected. They corrected the bridge graft and set it at the center of my nose bridge but either through a language barrier or a lack of technique, they never addressed the tip, though I told them about it. And for this reason I had set out to find a surgeon for my second revision on the nose tip. Dr Gantous isn't advertising his practice all over social media, he is a very specific surgeon that specializes in nose revisions, a surgery it seems that other surgeons do not want to do. That was very impressive to me and gave me great confidence to meet with him for my revision. And as I have first hand experience in the stereotype of seeking a surgeon that specializes in patients of your ethnic background, let me tell you that I now know this is absolutely not true. A skilled surgeon knows how to operate and achieve that best aesthetic result for any nose, of any ethnicity. And this is true of Dr Gantous. Back to my revision, when I met with Dr Gantous, I felt immediately confident that I found the right surgeon. Revision surgeries are a bit exploratory in that a surgeon wont be able to know 100% of what they have to work with until you are on the operating table but I had great confidence in what he told me he could see through his initial assessment and how much he could accomplish there. The surgery went smoothly and I have been recovering for 3 weeks. As soon as I went home, I was already impressed that I no longer had an ugly deep sunken scar across my columella and I could see my nostrils were oval and not round circles. I was even more pleased after 1 week when my bandage came off. It's still very swollen but I had no problem going out in public with my new nose 1 week post op. That's how much more of an improvement it was to my old nose. At 3 weeks I am over the moon with how my nose is healing. It's still a bit swollen but perfectly symmetrical to my face and proportions. Nose revision surgeries are not like primary rhinoplasty surgeries, it's difficult to even find a surgeon willing to do them. If there are potential patients of nose revision considering looking overseas because they are unsure of western surgeons, I encourage them to seek a consultation with Dr Gantous before looking overseas. I had found that South Korea does great primary rhinoplasty (with silicone) and blepharoplasty (they even do great revision work with eyelid) but nose revision isn't their specialty. You need somebody like Dr Gantous who spent his entire career fixing other surgeons' bad noses. He is a meticulous expert in the nose. I am so happy my path eventually crossed to his surgery. I am so glad that through this long rhinoplasty journey, I finally have the nose I've always wanted. I only wish Dr Gantous did my primary rhinoplasty to begin with. :)

I cannot say enough about my experience with Dr. Gantous. From the day I came in for my consultation for breathing issues and a deviated septum.. he made me feel safe and comfortable with the procedure that he was about to do. I won’t get into full details about my surgery but it was a Septorhinoplasty with a rib graft among a few other procedures. The process has been amazing .. from day one with the receptionists and then day of surgery with Dr. Gantous’ Nurses … I was very nervous on my day of surgery but they made me feel safe and comfortable and treated me like one of there own.. this surgeon has a very gentle comforting way of dealing with his patients. I just got home from my two week preop and for the first time in 10 years I can breathe properly through both nostrils! I cannot say enough about this man .. he brought me tears of joy today and I cannot thank him enough. If you or any of your family members or friends need surgery from this doctor he is definitely your man you will not be disappointed.

Did such a fantastic job that now I don't even pay attention to my nose

Dr. Gantous is one of the kindest, thoughtful individuals I have met in the field of plastic surgery. Since early childhood I have had a crooked/twisted nose due to an accident. It was bad enough where only 1 nostril was noticeable at majority of angles. Throughout my life I have felt very comfortable with my appearance other than when I came to realize I had a crooked nose when my eyes were corrected. After that, there was not a single moment where I had not thought about my nose, there was not one moment where I looked in the mirror and was comfortable with my face as my nose continuously attracted unwanted attention. After visiting multiple other rhinoplasty and ENT specialists within the GTA, Dr. Gantous really stood out to me because of his honesty and character. He spoke openly about attainable results and understood that I didn't want drastic changes, rather I wanted to compliment the rest of my face. He really made me understand the expectations to have so miscommunication did not lead to disappointments. Prior to booking, I spent 6 months deciding who I wanted to operate on me and Dr. Gantous was the only doctor who exceeded my expectations by having so much patience and answering the numerous questions I had in a timely manner. He even surprised me on the morning of surgery as he brought in an item of a hobby we both are very passionate about. Following a week of recovery which was completely doable, I visited him in his office for cast removal and was I shocked...my nose was completely straight but still recognizable.
