40 years old no kids but was overweight at one point. I currently have 15 year old silicone implants above the muscle bra size 32DDD (500cc I think)and want to have a breast lift plus implant exchange but UNDER the muscle this time and maybe go a little smaller this time. Do you think that galaflex (or something else?) is advisable to maintain results for longer?

You would do best to meeting with a board certified plastic surgeon and try on a variety of sizes to see what you like. Bras are not all made to standardized sizes.
It appears that you have suffered some stretching of the scars. A more extensive lift would better distribute the tension and should allow for improvement in the scars. A "lollipop lift" should be adequate. A more definitive opinion would require a formal examination.
The medial pockets could be tightened so that the implants cannot come so close together. You will not have quite as much cleavage, but would look better.
