Yes, insurance is required to cover the costs of reconstruction, including implants. If you desire, most companies also cover surgery on the opposite breast to obtain symmetry. Please go visit with a board certified plastic surgeon or breast surgeon who does reconstruction.
As far as I know, breast cancer patients are able to get their breasts reconstructed at any time and insurance covers the surgeries. Many patients choose to delay their reconstruction and I have never had an issue with insurance companies covering procedures for breast cancer survivors. I would schedule an appointment with a surgeon to get more information and to start the process but I do not believe you will have any issues. You will probably need to supply your medical records so you may want to get those before any appointments just to help expedite the process.
It is common for the breasts to change with pregnancy and childbirth. However, to be on the safe side, it would be a good idea to get your breasts checked by your doctor.
