Pre-op: BWD 9cm, Weight 100lbs, Height 5'3", 28" under-bust, 32" bust. Post-op in surgical compression bra, bust 34". Only 1 day post-op but did not expect this large increase in size with 200cc silicone (Allergan style 45) under muscle (wanted maximum 1 cup size more, I like my small frame). Will the swelling go down lots? I understand it's early, but is everything going to settle and shrink? Will the implants flatten out under the muscle & look like natural breasts eventually?
Answer: Implant volume and cup size
At size 34 each 100 cc’s of implant corresponds to 1 cup size change. 200 cc implants should take you up 2 cup sizes. After surgery, there is a certain amount of swelling and this will go down in time.
Best Wishes,
Gary Horndeski, M.D.

Answer: Sorry, but I think this was a bad implant choice!
I see that you are short and quite slim, but I would doubt that I would measure your BWD as 9cm. Every surgeon does this differently, since breasts do not have "sharp" beginnings or ends." But in your case this apparently led to choosing the narrowest-base, highest-projection implants available in the size you requested (and needed for a 1-cup increase).
I think the size choice is close to adding a cup size, but this implant will always look like a baseball, not a breast. You don't have too much size increase, you have way too much projection, which looks even bigger when they haven't dropped yet!
Yes, it is early, but I hope you and your surgeon have already discussed revision policy and who pays for what. I hate to be overly critical, but this sounds like a rookie misjudgement, and even with time and swelling resolution, this implant won't change and it certainly won't flatten. (Without seeing you) I might have recommended something in the 250cc moderate (maybe moderate plus profile if you really are that narrow-chested) range. Too small and you could also end up with a wide, unnatural, "Grand Canyon" cleavage due to too-small implant diameter.
This is an example of a single (incorrect) measurement (BWD of 9cm) being used to select implant profile when the pocket base was likely closer to 12 or 13cm or more. Again, without being there, it is impossible to state this dogmatically, but the (bad) results speak to this analysis. Sorry. I wish I could be more hopeful, but I think you will need re-operation, and I might consider consultation elsewhere if this exemplifies the judgement used. Best wishes! Dr. Tholen

Answer: Early Breast Augmentation Results
Everything you have mentioned is very typical for early after breast augmentation surgery. You just need to let swelling go down and get used to the current implants for three months and then decide if they look natural and are of the right size. Despite all of the measurements and breast implant size options, no one can predict with unerring accuracy how any patient will interpret the final result. You can have to wait until you get there before making a decisive judgment.
