Hello, I wanted to share my story with any others interested. I am male to female transgender, 25 years old, 5'1" and 135lbs. I started my search for a plastic surgeon for my Breast Augmentation surgery as the next part of my transition around January of 2017. I have been on hormone replacement therapy for over 3 years at the time of initial consults and was only at about 34A in size. I was definitely not happy with the size. I looked around for several surgeons in the Long Island area and started scheduling consults. Most made me wait 20-30 minutes in the lobby and then another 5-15 in the office. It became a pattern. The doctor comes in to do the consult and takes a 5-second look and starts rattling off what is needed and immediately tries to pressure a date to be booked for procedures. I was quite dissatisfied with the experience I had at first. Some weren't even all to concerned with what I wanted and just sad they would do what they would when the operation was happening. My final consult was with Dr. Dobryansky of Long Island Plastic Surgical Group who I found through this website. The office was very modern and clean while the receptionists were extremely professional. I waited less than 2 minutes before being called into the office and immediately started the questioning process I had become so familiar with. After the questions were done the doctor came in right afterward to take a look. I was extremely impressed with their patience. I had brought a list of questions with me and they answered every one of them with detail. They even did vector imaging right on the consult to show me what it could look like with each sizing option. The doctor himself spent over 40 minutes with me during the free consult with no pressure to book a date right away. After several consults all over Long Island, I was 100% satisfied with this one. A week later I booked my surgery date for late August. I was quoted a price of $8,000 for the procedure that included everything: Operating room, anesthesia, pre-op testing, post-op care, medication, post-op appointments, and even a year-long agreement that if any revision surgery were needed it would be covered (although unlikely needed). I later was able to get my surgery covered by insurance as part of a gender identity medical necessity. So far everything, including medication, has cost me around $1,300. The surgery itself took place at a small hospital and the staff was very nice and professional. The procedure took about 3 hours. I had 355ccs silicone implants done. There wasn't much pain but quite a lot of discomfort with the swelling and sore muscles. I was able to return to work after about 2 weeks, although if I had a desk job it would probably have only been a week. I was extremely happy with the results that I had. Originally I was told I might need a lift on my left breast (which several surgeons just ended up quoting me a whole other price tag for), but it was included in my original single price for the augmentation. In the end, I did not need it as Dr. Dobryansky was able to even things out with implants alone. My only regret is that I wish I had gone a little bigger. The vector images are a little deceiving but I am nonetheless extremely happy with what I got! Although my insurance covered it, in the end, I would have had no problem paying the original price if needed! It was well worth it! Updated on 28 Oct 2020: After a few months of having the implants I noticed I had a double bubble effect going on. This was due to me having very little fat on the bottom part of the breasr tissue. I had more fat on the top half which made it uneven. Dr dobransky offered to fix it for me too but I had this procedure done through insurance and at the time this came up I had already lost the insurance that covered it. This wasn't a huge issue as I knew it could be fixed but I was saving for my facial surgeries and BBL so I put it on hold as no one would ever see this common complication but me. My boobs look great in bras and dresses but fixing this will purely be for my own comfort :) Now that my BBL is done I will be looking for a revision with him but I will be changing the implants to ultra high profile and may even opt for them to be over the muscle to avoid this issue in the future. I may go slightly bigger and go for 450ccs instead of 350 too!
I've always wanted a tummy tuck. I finally said it's time for me. For the last few years I attend the gym a few days a week, run around with my kids at sports, and do everything for everyone else. I looked around and went for a consult at Long Island plastic surgery group. The doctor and office staff is great and I'm excited to move forward. I am scheduled to get a tummy tuck with suction assisted Liposuction to my abdomen and flanks.
Thank you for your question.
Every surgeon is different when it comes to post-operative care, so it is important to ask these questions. It is certainly reasonable to undergo a cosmetic procedure without the use of opioids if you are concerned. With my patients, I use a long-acting nerve block called Exparel which tricks the body into thinking you didn't have surgery for up to 72 hours. Most of my patients are able to get by with just using extra strength Tylenol. If something of additional strength is needed, medications such as Tramadol can be prescribed. I recommend speaking with your plastic surgeon to discuss your concerns.
With breast augmentations in patients who are very active like yourself, I recommend placing the implants above the muscles. This will reduce the implant shifting into the axillary region as well as the pectoral muscle animation. I recommend speaking with a board certified plastic surgeon to discuss your options.
Based on the photos provided, I believe that liposuction to your flanks would give you the snatched, hour-glass figure you are describing. I recommend speaking with a board certified plastic surgeon to discuss your options.
With any breast implant procedure, you can expect discomfort and swelling intermittently within the first year. New activities such as upper body workouts, laying on your stomach, and even coughing can cause a short period of additional discomfort as the pectoral muscles are working with the added tension of the implant depending on if the location is above or below the muscle. If you are experiencing extreme discomfort, I recommend speaking with your plastic surgeon about your concerns.
Seromas, or fluid build ups should ideally be drained by someone with expertise in this area. This can be your surgeon, a PA, a registered nurse, etc. If interventional radiology is necessary, your doctor can refer you to a specialist who can assist you in this procedure.
