This Orlando, FL woman was bothered by breathing issues, the dorsal hump on the bridge of her nose, a drooping nasal tip, her recessed chin, and excess skin under her chin. Dr. Gross recommended Rhinoplasty, Chin Implant, & Submentoplasty. to create harmony with her 3 main areas of concern: nose, chin, and jawline. Her nose now has a slight slope along the bridge with an upturned tip. The vertical lengthening chin implant (from Implantech) evens out the proportions of her face in profile, and the chin tuck (platysmaplasty) provides definition to her jawline and tightens the muscles in her neck. Patient treated with Cheek Implants, Rhinoplasty, Brow Lift

This 26-year-old cosmetic patient presented in our plastic surgery office requesting consultation about multiple facial procedures. She wanted correction of the nose but also improvements around the eyes and possibly bigger chin.After detailed analysis of her facial structure, both bones and soft tissue, we concluded that patient would benefit from augmentation of chin and cheeks with facial implants, nose shaping surgery and fat grafting over cheeks and around the eyes. After thorough discussion we concluded to proceed with rhinoplasty, cheek and chin implants and fat grafting.Her surgery was done in general anesthesia in our office based plastic surgery center. It consisted of external rhinoplasty (nose shaping), chin and cheek augmentation with implants and fat grafting over augmented areas of chin and cheeks and around eyebrows. She was discharged home 1 hour after surgery. She left country on a sailing boat 2 weeks later.Patient is shown before multiple facial implant surgery and nose surgery on the left. Postoperative images on the right show patient 6 months after chin and cheek silicone implant augmentation, nose surgery and fat graft placement. Notice how her elongated, bonny, ascetic face turned into fuller, oval shaped, more harmonious one after multiple facial surgeries.

For lower infection risk versus an intraoral approach, implants were placed transconjunctivally and were fixated with two screws to avoid implant mobilization risk.

This 48 year-old woman requested cheekbone enhancement as she felt this area of her face was too flat and without shape. Dr. Tholen performed cheek implant placement (solid silicone anatomic malar shell) via lower eyelid incisions. Note the improved definition of her cheekbones in the post-op photos (as well as the absence of visible scars). Intraoral incisions are often touted as being scarless but the potential for infection of the implant(s) via mouth bacteria is unacceptably high, in my opinion, especially when the eyelid approach allows rejuvenation of the lower lids (if needed) at the same operation. She also had fat excised beneath her chin in the upper neck area at the same operation, defining her jawline better and slimming her neck. After photo is 3 months post-op.
