This patient had previous implant-based breast reconstruction after breast cancer and was unhappy with her breast contour. She was particularly concerned with the divot above the left breast implant. She elected to undergo fat transfer to add natural- volume and a softer looking aesthetic result. Here are her results after one round of fat transfer. She plans to undergo addition fat transfer to further build natural volume to improve contour in this area. Fat transfer was performed by re-purposing stubborn fat from the waist area. This fat was removed using liposuction techniques and added over top of the previous breast reconstruction.

Our patient underwent a breast implant removal (explant) with total capsulectomy in 2021 for severe autoimmune issues and pain related to the implants. She recovered well with significant improvement in her health. She had cosmetic concerns with breast deflation and concavities secondary to severe tissue stretch from the implants. I performed liposuction of her abdomen, flanks and thighs with Fat transfer to the breasts to contour and volumize them.

She lived deformed for 13 years until she found Dr. Rad. Dr. Rad removed the implant and rebuilt her breast with a left DIEP flap and revision breast lift.

Breast Reconstruction Revision for this 60 Year Old Woman

Breast Reconstruction Revision for this 52 year old woman who failed implant-base reconstruction. She had a history of radiation therapy and painful capsular contractures. Her implants had been removed. She came to me seeking breast reconstruction with her own tissue.  Bilateral DIEP Flaps for breast reconstruction and nipple areola reconstruction were performed.  The radiation damaged skin needed to be removed. She is pleased that she was able to complete breast reconstruction.

Breast Reconstruction Revision with DIEP flaps for this 52 Year Old Woman

Patient initially underwent breast reconstruction in the 1980s. She had undergone several revisions for recurrent capsular contracture, most recently in 2004 with placement of new silicone implants. She presented with progressive capsular contracture with hardening and distortion of her implants, obliteration of her breast fold with bottoming out of the implant, asymmetry, and overall dissatisfaction with her result, wanting to be smaller overall with improved appearance, shape, and symmetry. She elected to undergo revision of her reconstruction with removal of implants, complete capsulectomy, revision of the implant pocket, placement of Allergan 410 MX 550 cc shaped silicone implants, as well as new pocket creation and reinforcement with Alloderm (acellular dermal matrix, ADM).

The patient was dissatisfied with her breast reconstruction. She underwent revision breast reconstruction with repositioning her folds, tailoring of the skin and implant exchange with shaped implants

This patient previously had a lumpectomy with radiation down on the right side. Unfortunately for her she had some Aesthetic complications as a result, the issues were related to post radiation scarring along with a situation where the scarring was pulling the nipple off to the side. After having a detailed discussion with this patient and figuring out her wishes and desires we proceeded with a Breast lift/reduction with lipocontouring on the left side to help with symmetry.Once this was done we then proceeded on the right side by reconstructing the defect, this involves excising all the old scar in radiated area, once this is done a substantial defect is created. The only way to then fix this is to move or rotation nearby tissue into the defect, this preventing irregularities or defects. This technique is often described as an Oncoplastic Reconstruction. As you can see from the postoperative photo there is significant improvement. Please keep in mind that the right side which is radiated will always be a little bit higher and asymmetrical. The effects of radiation are for life. However there are work arounds that a surgeon can do to help improve symmetry as seen in these photos.

This 53 year old female had a history of breast cancer with mastectomy, radiation therapy and implant-based breast reconstruction. She failed breast implant reconstruction. Her old scars, radiation skin and implant were removed at the time of bilateral DIEP flaps for bilateral breast reconstruction. She also had bilateral nipple areola reconstruction. Nipple tattooing is an option if she wishes.
