Q: Strangely shaped nose, can the bone be fixed with fillers so I can only have surgery for the tip? (Photos)
Answered by Dr. MontagueApril 10, 2024

A: Great question. From your pictures it looks like your nasal bones are wide in the area we call the rhinion. I would not recommend addressing that with fillers especially since you are considering tip surgery anyway. I completely understand that you would like as little surgical intervention as possible and all else being equal I think that is a great mentality. However, in this case I do not think fillers would be effective in treating this area of your nasal bones. Plus, if you are having surgery anyway it is fairly straight forward to address the nasal bones surgically at the same time and not expose yourself to untoward risk or extra healing. Ultimately, surgery for the nasal bones would not only give you a better result than would fillers but also a result that is permanent. Hope that is helpful!

Q: Can a neck lift fix my obtuse neck? Or is a low hyoid causing it? (photos)
Answered by Dr. MontagueApril 8, 2024

A: An in person physical exam would be necessary to say definitively but I do think a deep neck lift and/or facelift could help obtain your desired look. From the pre and post X-rays you sent it looks like your surgery was successful and probably did help your jaw and neck line aesthetics. However, a deep neck lift would allow you to sculpt underneath the platysma in order to create an even higher, tighter jaw line. I do not think your hyoid is in a poor position so that shouldn't hold you back. Hope that helps!

Q: Alternatives to a facelift 2 years after FaceTite? 41 y/o
Answered by Dr. MontagueApril 8, 2024

A: That's a great question. I would say I partially disagree with the other doctors you consulted. Certainly any of the "energy modalities" like facetite can affect the vascularity of the skin, and can certainly cause some scarring beneath the skin that causes the dissection for the facelift to be a bit trickier. I have seen that many times. And while theoretically you may be at a slightly higher risk of skin necrosis due to the previous facetite procedure, a facelift surgery is by no means off the table. You may want to consider a deep plane facelift as classically this approach has less vascular risk. Obviously a full evaluation and medical history would be necessary before making any decisions but from what you've told us in the question I would say a facelift is still an option. There's hope!

Q: Can I do rhino, lip lift & lower blepharoplasty all in one?
Answered by Dr. MontagueApril 4, 2024

A: Great question and actually something I run into quite a bit in my practice. In my opinion performing a lower blepharoplasty at the same time as a rhinoplasty is perfectly acceptable. However, I prefer not to perform lip lifts at the same time as a rhinoplasty because the incisions are very close and could risk very poor skin healing/skin necrosis. Reasonable minds can differ but that is my medical opinion. In these situations I would stage the lip lift after the rhinoplasty. And for what its worth, as far as saving on the anesthesia costs, the lip lift portion is something that most people can easily tolerate under local and therefore you could avoid the anesthesia cost altogether. Of course, your surgeon would have to offer lip lifts under local. Just a thought. Hope that helps!

Q: Is it better to do all face procedures at the same time?
Answered by Dr. MontagueApril 3, 2024

A: This is a great question and one that I get asked multiple times a day by patients. In fairness I don't think there's one right answer but when able I prefer to combine these procedures. Sometimes, due to medical concerns, the length of the procedure may be prohibitive such that you'd want to split up your procedures. That's a decision for you, your surgeon, and the anesthesia team to make. However, from an aesthetic standpoint I think combining these procedures is perfectly acceptable and you get the advantage of one period of healing. The other advantage is that, assuming you were a candidate for all the procedures you mentioned, you would be addressing the upper, middle, and lower face all at once which is better for facial harmony. Hope that helps!
