Is there an implant that would give me porn star fake looking boobs? Without bra I want them high up, close and full (Photo)

It is very difficult to determine the exact size and shape implant you will require to best match your ideal breast "porn star" image without an examination by a board certified plastic surgeon. Not just any board certified plastic surgeon, but one with many years of frequently performing breast augmentation surgery including different approaches, techniques and implant choices. This is because several measurements not to mention your breast characteristics are needed to determine the optimal implant size to obtain your goals. Without knowing these dimensions it would be difficult to make this determination. For example, the existing base width of your breast will determine, in many cases, the maximal volume per implant profile that you can accommodate. To illustrate; a 100 cc difference may make a significant difference with a narrow base width breast, but much less of a difference if you have a wide chest wall and wide breast “foot print”. Therefore, just because your friend may have a great result with let’s say a 350 cc implant to make her go from a “A” cup to a “C” cup size does not mean that you will have the same result with the same size implant. . Further simply placing implants in a bra to determine the size best for you is not always accurate as the bra often distorts the size, is dependent on the pressure the bra places plus the implant is outside your breast and not under it among other variables. Computer software morphing programs that automatically determine the best implant size can be helpful in some but not all cases (e.g. doesn’t work well in my experience with existing implants, sagging or asymmetric breasts). Using “want to be” photos however are useful if simply provided to the surgeon as I will further explain in the below reference.

To have "fake" looking breasts the size of the implant would have to be much larger than your body type. That will also stretch your skin and breast tissue beyond its capacity and may exaggerate the concern you have of them falling too much to the sides when you lay down. Even if you close the implant pocket on the sides, a larger and heavier implant will stretch the repair and have the same issue recur. Larger implants consistently have more downside compared to a more proportionately sized implant. However, you can discuss a wider implant with your surgeon which can span your chest better but you do lose projection. There may be a compromise that will suit you. I would recommend discussing with your board certified plastic surgeon. 

Thank you for the question and pictures.  Based on your photographs and description, your goals may be more closely achieved with the use of capsulorraphy techniques;  this would involve closure of the breast implant pockets on the sides and opening of the breast implant pockets towards the cleavage area. You might want to consider a lift (mastopexy) with high profile saline or silicone implants to get you more of a "porn star" look.  Your natural breast anatomy, however, seems to place your breasts with a wider central gap.  Therefore there is only so much a revision surgery can do for you.  Best of luck to you 

Remove and replace implant procedures are very common.  Most women are now exchanging their saline implants for the more natural feeling and looking silicone implants.  The newer silicone implants have been FDA approved since 2006 and offer more sizes and shapes than the now outdated saline implants.  They come with a lifetime warranty that also covers surgery for implant capsule formation.In this situation, I would recommend switching from saline to silicone and using a larger, high profile style, implant to give the projection requested, something between 700-800 cc's.  I would also switch from below the muscle to on top of the muscle to obtain the desired look.Implant exchange procedures are typically much less painful than the original procedure with a 2-3 day recovery.  Make sure you visit with a board certified plastic surgeon who performs exchange procedures frequently.Good Luck!

Some requests could be accomplished by larger implants and above position. But your anatomy and genetic breast will never be close together. Sorry. 

All implants do is make a bigger version of what you bring to the table. If you try and alter this by dissecting too close in the middle you run the risk of synmastia (uniboob). The fakest looking breast are capsular contracted breasts but that condition is frequently painful. Your tissues seem to be having trouble supporting the implants you have so changing your expectations to something more reasonable may be in order.

This is a form of what is referred to as the "Michael Jackson problem". You can take what you have and make certain changes with minimal risk but it is not possible to have whatever look you might like just because you ask for it and find a plastic surgeon who is willing to do it. Basically a "fake" or "porn star" look with implants is because the implants were too big for the breast they were filling, the implants are not positioned properly in relation to the breast base, the implants are above the muscle, and the true base of the breast is distorted in an attempt to move them closer together or move them up. A plastic surgeon interested in controlled and minimal risk results would not agree to do this but celebrities or those with enough money can usually find someone who will do it for them.
