I want a chin implant, the Mittelman Pre Jowl from Implantech. Which size? Large or X-Large?

Without an exam your question cannot be answered.  You will need to see an Oral/Maxillofacial surgeon to see if othognathic surgery, genioplasty or using an implant is best for your specific case. It should be noted that patients with severely weak jaws or abnormally functioning jaws may be better treated with other facial surgery (orthognathic surgery), although a chin augmentation may be an option at a later time. In our practice, we have used Implantech Mittleman solid silicone chin implants for over 25 years with an excellent success rate. In our practice, we use size small or medium all of the time. We have never used a size larger extra-large in over 25 years.  The size and the shape of the implant is determined at the time the consultation, examination, and computer imaging.

The difference between large and extra large in this chin implant style is 1.5mms. That is not really a very noticeable difference. However in almost any form of facial implant implant when in doubt go with the 'smaller' size. Many more revisions are done because facial implants are too big rather than too small.

Based on your photos, I would recommend the large.  Your surgeon will measure and get a feel for what you are trying to achieve.  Best wishes for a successful surgery.
