Your photos suggest that achieving a more defined and natural-looking V-shaped facial contour can be effectively accomplished with a silicone chin implant, which is biocompatible and designed to provide a natural enhancement. The silicone chin implants that are currently used have a wide extension bilaterally that eliminates the appearance of the witch's chin in the frontal view. The anterior projection may increase and this procedure is much easier than a genioplasty and certainly less complications. A chin implant can be removed or changed if necessary.

Genioplasty is an effective way to alter your profile and I do many of them. However, it is more intensive and I disagree with your surgeon regarding the relative risks. In fact, I think that chin implant is less invasive, has a lower risk of infection, and can be more easily adjusted. You will certainly have swelling for much longer than one week with a genioplasty while chin implant can be done right in the office.

Not a chin implant, firstly for their null effect in increasing the vertical height (your primary need), and then for the high rate of complications making them always short-lived for the patient. They are appealing: quick surgery, cheap, outpatient management and veeery easy technical for the surgeon, even the less skilled and experienced can do them, however they have very bad downsides, in practical terms they have short duration due to pain, displacement, visibility, mobility, weird gestures, failing to achive goals, etc, and I mean this about silicone, medmore and any other material used for them.

You have microgenia, this is small bone developtment at the chin, therefore you need chin augmentation with bone supply (iliac grafting); not a case for sliding genioplasty giving there's nothing to slide being your chin osseous mass in shortfall, it is primery deficit of bone substance; sliding is only for cass with misshapen bone at the chin with no specific bone mass deficit.

Your chin needs a significant increase in vertical height to balance your facial proportions, with a moderate (to prevent masculinization) forward augmentation. This can only be done with bone grafting in between the ostetotomies planned, the donor site is the iliac bone (hip). This requires no interlocking of teet, no jaw fracture, no straw nourishment, you can speak, chew and eat normally.
