I had upper and lower months ago. The brow skin covers the half of the eye and is drooping over. The hair reaches the incision. I force closure during the day & keep them shut as often as possible. At night I tape them. With them open & my face fully relaxed, I look drugged. When my face is held normally there is no/little lid show. A top ophthalmologist said plugs and grafts. Oculoplasts say it's "too complex". Is it? The original surgeon is not interested.
Answer: There is help without skin grafts. Skin grafts are often proposed by surgeons who really do not understand the underlying pathology in these post surgical eyelids. There are circumstances where skin grafting might be appropriate but every effort should be make to reconstruct the eyelids without a skin graft. The grafted skin is unsightly and is stiff. As a result, the aesthetic restoration of the eyelid is completely unsatisfactory. You appear to have enough skin to close the eyelid. It is much more likely that scar tissue that developed from the blepharoplasty is tethering the upper eyelid internally and limiting closure. By braking up this scar tissue, it is possible is these cases to improve closure. In some of these situations, raising the lower eyelids can also help to cover more of the eye and protect the eye surface. What you might precisely benefit from is something that can only be determined by a detailed in person assessment. Yes, many of my colleagues do not understand the repair of these eyelids. I do agree with the use of plugs, artificial tears, and other dry eye measures. I am attaching video that shows how this work is done. I am also providing a website link with more information. You have other options. A skin graft is seldom the right answer.

Answer: Please label pre and post op More information is needed about your surgery. I would assume that the far left black and white photo is pre op and your photos looks like very recent post op although I can't tell how far out you are. It looks like ptosis post op assuming your photos are at leat one month post op. Its best to consult with an oculoplastics specialist who performs external ptosis work.

Answer: Waiting Based on these photos, your situation does not look so desperate that immediate intervention is needed. Stretching the eyelids would improve this condition in time and surgery may not be needed. Taping and placement of ophthalmic ointment over night will help. It this does not correct within next 6 months, skin graft placement should be my suggestion. Good luck.
