Asymmetric and downturned eyes - what surgeries would you recommend for me? (photos)

As we age, our eyelids naturally sag and droop. A combination approach would be best to see visible improvement. In terms of non-surgical treatment options, I would recommend a Chemical Brow Lift (using Botox, Dysport, Xeomin, or Jeuveau) and filler injections to rejuvenate the temples, eyebrow, eyelid, eye troughs, upper eye hollows, and forehead. Thank you for submitting your question and photo. The eyelift procedure known as blepharoplasty can correct heaviness in the upper lids and define the crease to reduce the downturned appearance you describe. The shape and curve of the eyelids can be contoured for a more awake and symmetrical form.
This minimally invasive surgery can also diminish darkness under the eyes by improving facial tone in this area. Together, the steps of this procedure can create a volumized, healthy, and symmetrical eye result. You should be seen in person for eyelid concerns to be properly evaluated, and to see your whole face which affects the surgical plan. Also, know that all faces are asymmetric, and the slight differences between your eyelids are normal. Based off of what I see here, I would recommend an upper blepharoplasty, canthopexy, and fat grafting to the cheeks and around the eyes to address your concerns.

You may benefit from lower eyelid retraction surgery with canthoplasty but need more detailed evaluation and discussion with expert to determine.

Hello. You have low eyebrows and ptosis of the upper eyelids. Once these are corrected, the upper part will go up and the visibility of the lower part will increase even more. This is why canthoplasty is necessary. However, the power of canthoplasty alone cannot correct the collapse in your midface.
