I am 11 months post-op a transcutaneous lower blepharoplasty for malar bags (unsuccessfully) and have a (I feel) pretty visible scar under my left eye. It’s not a keloid just uneven and the area dabove the directly above the scar is discolored. I have been scar tape, scar gel, and numerous oils. I am a fitzpatrick skin type v. What can be done?
Answer: Options for Improving Post-Blepharoplasty Scarring in Fitzpatrick Skin Type V I'm sorry to hear about your concerns following your lower blepharoplasty. Here are some potential options to address the visible scar and discoloration under your left eye: Laser Treatments: Fractional Laser Resurfacing: This can help improve the texture and appearance of scars by promoting collagen remodeling. It's important to choose a laser that is safe for Fitzpatrick Skin Type V to minimize the risk of hyperpigmentation. Microneedling: This treatment can help improve scar texture and promote even skin tone by stimulating collagen production. It can be combined with platelet-rich plasma (PRP) for enhanced results. Chemical Peels: Superficial chemical peels may help with discoloration and improve skin texture. Ensure that the peel is suitable for darker skin tones to avoid complications. Steroid Injections: If there is any raised or thickened scar tissue, steroid injections may help flatten and soften the scar. Topical Treatments: Continue using scar gels or creams containing silicone, which can help improve the appearance of scars over time. Consider products with ingredients like vitamin C or niacinamide to address discoloration. Camouflage Makeup: While not a permanent solution, high-quality camouflage makeup can effectively conceal scars and discoloration for daily wear. Consult with a Dermatologist or Plastic Surgeon: Given your skin type and specific concerns, it's important to consult with a professional who has experience treating scars in patients with Fitzpatrick Skin Type V. They can provide personalized recommendations and ensure treatments are safe and effective for your skin type. It's crucial to approach treatment cautiously to avoid further pigmentation issues. A professional evaluation will help determine the best course of action tailored to your needs.

Answer: A light co2 can be done under the eye. Our office has many different treatments for scars depending on the depth of atrophy or hypertrophy. Scars require a combination approach using our HI DEF protocol which uses a combination of lasers, topical scar modulation, and RF treatments to pixelate scars, improve contour, reduce hyperpigmentation, and improve the visibility of the scars so that they are no longer visible in ambient light. Everyone has a unique pattern of scarring and we employ our HIDEF technique to improve texture, color, contour, and topography. We often use a combination of treatments that begin with a careful examination to examine the micro texturing of the acne scars. Atrophic scars and indented scars can benefit from injectable fillers such as Radiesse, Sculptra, and Restylane with or without subcision to remove tethered scars and atrophic depression. Hyper-redness in scars can be improved with V beam laser, Excel V, BBL, IPL, and Venus RF to improve the redness and reduce the visibility of deep scars. Fractional Thulium laser and clear & brilliant can also reduce the inflammation and redness of the scar when combined with topical therapy. Fractional lasers such as Fraxel, erbium, and CO2 laser can be combined with radio frequency and also fractional RF to improve textual issues and microtopography. Subcision and TCA cross are also used for deep ice pick scars and narrow valley scars in order to improve the micro texture. Above all, our combination approach with our clinical nurses and board-certified plastic surgeons can improve the quality of your skin by using at home Melarase creams combined with PRP treatments in our office to improve your skin quality and tone. I recommend getting a formal evaluation with an experienced team of experts in order to reduce your scarring. Finally, hyperpigmentation of scars can be improved with at-home Melarase AM, Melarase PM, and Melapads to help reduce active PIH and melanin deposition. Best, Dr. Karamanoukian Realself100 Surgeon
