What is best for me a chin implant revision or sliding genioplasty? (photos)

Question

I'm 37 yo F, I had a silicone chin implant 12 yrs ago (size 1 Allergan symmetric) Never had a problem, but was always self-conscious that it was too small. Last year, I upgraded to an ImplantTech XL anatomical Implant. Immediately it looked great, but after a few weeks it was sagging out of position, creating a strange shape. My physician says my periosteum is non-existent from the surgeries and there is no tissue to attach an implant. Is there hope for me? Sliding genioplasty/new implant?

You are a good candidate for a genioplasty. This is a permanent procedure that allows us to pretty much put your chin in whatever position you like. By bringing it slightly forward and down, this would create more definition to your jawline and add balance to your profile. It is popular to see a chin implant replaced with a genioplasty, as this would get rid of the chin ptosis (sagging). I would highly recommend finding a surgeon who specializes in "genios" and set up[ a consultation!

Your chin implant appears to be hanging off the end of the bone. Such a chin implant malposition issue can be solved by screw fixation. The purported lack of soft tissue for suture fixation becomes irrelevant then.

The use of implants can lead to your situation and indeed, a less 'natural' look with aging. Ideally, the implant would be removed and the genioplasty performed to achieve the proper 3D look. Any scar tissue from the implant capsules has to be reviewed at surgery. Xrays always help to determine the best plan
