Your chin implant appears to be hanging off the end of the bone. Such a chin implant malposition issue can be solved by screw fixation. The purported lack of soft tissue for suture fixation becomes irrelevant then.

The use of implants can lead to your situation and indeed, a less 'natural' look with aging. Ideally, the implant would be removed and the genioplasty performed to achieve the proper 3D look.
