I’m 74 years old and have had my silicone implants for 40* years. Just found out they are ruptured but still inside the capsule. Trying to figure out where to go from here?????

Rupture silicone implants need to be removed because the silicone gel can spread. You may or may not need capsule removal. The technique I recommend is explantation and lift using the Bellesoma. The implants are removed, your breast tissue creating upper pole fullness, elevated higher on the chest wall and more medial to increase your cleavage. The weight of the breast is transferred to the underlying pectoralis major muscle resulting in long term stability. Vertical scars are avoided and nipple sensation (in 95%) is maintained if not injured during your previous surgery. Later, fat transfers can be performed if additional volume is desired.

A rupture that stays inside the capsule is not an emergency, but 40 year old implants that have ruptured should be removed, ideally with the surrounding capsule, since the capsule contains the silicone and is often thickened or calcified after this many years. The main decision is whether to replace them or not. At 74 many patients choose removal with a lift if needed, because it means no future implant problems, and breast shape is often better than expected once the old implants come out. Replacement is also possible if you want to keep volume and your general health allows it. An MRI or ultrasound beforehand helps confirm the silicone has not spread beyond the capsule. Please also make sure you have had a recent mammogram, since silicone can hide findings.
