Thanks Good4130 for your question! Surgery is typically the only option to get a predictable, semipermanent, or permanent outcome for someone who is particularly aiming for a half-inch projection or a bigger diameter.
A Board Certified Plastic Surgeon can increase the size of the nipple by using fat grafting, which involves transferring fat from another area of the body, or dermal fillers, such as Juvederm.
Plastic surgeons can perform structural surgery to "lift" the nipple tissue and increase its projection by using internal sutures or tiny grafts. The procedure is called Nipple Lift or Breast lift (Mastopexy) procedure.

The most predictable enlargement can be achieved using local tissue from areola to augment and elongate nipples. This will create visible scar(s) on areola which are usually fine. Use of patient own tissue as a dermal graft may also be considered and will provide longer lasting result than fillers, fat, etc. In person evaluation is recommended. Good luck.
