What causes nose necrosis? What is the chance of getting necrosis in a revision rhinoplasty?

Why do some people not get necrosis even if theyre on theyre 3rd plus rhino? What causes necrosis and how do doctors prevent that? Also, would you see necrosis right away or can it appear months or years after a rhino?
