I'm planning to have explant and replacement of my current old breast implants. I may go with slightly smaller volume. My ideal weight is 10 - 15 pounds less than I am right now. How important is it to get to my ideal weight first for the best aesthetic outcome?

Breasts have 2 components: breast fat and breast parenchyma. Losing 10-15 lbs. may or may not have any significance. Unfortunately, you have not provided any photographs or told us your bra size. The most important factor after explant is the need for a breast lift. The technique I recommend is explantation and breast lift using the Bellesoma. The implants are removed, your breast tissue is reshaped creating upper pole fullness, elevated higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained if not injured during your previous surgery. Later, fat transfers or small implants can be placed if additional volume is desired.

Hi there,
If you can realistically lose the 10 to 15 pounds and keep it off, I'd do that first. If your weight has been steady for a long time and that's where you'll likely stay, it's reasonable to go ahead now.
A few things worth knowing:
- Breasts often change with weight, especially after years with implants. If you lose weight after surgery, the breast can deflate a bit and sag more, and that may change the result we planned.- Going slightly smaller with your new implants leaves a little extra skin. Add later weight loss on top of that, and the chance that you'll want a lift goes up. Planning around your stable weight helps us decide if a lift is needed now.- Being at a healthy, steady weight also slightly lowers the risk of complications in general.- Honestly, the key word is stable. A crash diet right before surgery doesn't help. Reaching a weight you can hold for a few months does.
An in-person exam is needed for personal advice, since your skin quality, current implants and nipple position all play into the plan.
