I am fine with how far out my chin projects, but it projects too far downward, especially noticeable when smiling. Just was curious whether this procedure may be able to change that.

Yes, in many cases genioplasty can change not just how far the chin projects, but also the direction of that projection. If your concern is that the chin comes downward more than forward, that is something that can often be addressed with surgical repositioning of the chin segment.

In general, genioplasty is a very versatile procedure because the chin bone can be moved in more than one dimension. That means it may be possible to reduce vertical length, rotate the segment, and improve the overall contour so the chin looks less downward-heavy, particularly in profile. For patients who are comfortable with their forward projection but dislike the vertical drop or the way the chin behaves when smiling, that distinction is very important.

Smiling does add another layer to the evaluation. Sometimes what appears to be a downward-projecting chin is related not only to the bony anatomy, but also to lower lip position, mentalis muscle activity, dental show, or the way the soft tissues move dynamically. In my experience, that is why an in-person exam is so valuable. A still photo is helpful, but animation and facial movement often tell the fuller story.

Based on your description, it is certainly reasonable to ask whether the chin can be reshaped rather than simply brought back. In many patients, the answer is yes. The goal would be to create a chin that feels more balanced and less vertically dominant, while preserving a natural masculine profile. Remember, chin reshaping is less predictable than genioplasty.

It would be worthwhile to consult with a board-certified plastic surgeon or facial surgeon who is experienced with osseous genioplasty and profile analysis. Imaging and a proper examination can usually determine whether the issue is primarily excess vertical height, the angle of projection, soft tissue dynamics with smiling, or some combination of these factors.

In your photo, it is not possible to properly assess chin projection due to the beard coverage. Facial hair can significantly obscure jawline definition and chin position.
A clean-shaven photo showing your full face from both side and frontal angles would allow for a more accurate evaluation of projection, balance, and profile harmony.
Only with clear visualization of the chin–lip–nose relationship can appropriate recommendations be made.
