Blue vein under eye after lower blepharoplasty with fat transfer? (photo)

This is not very common to see after fat transfer to the lower eyelids.  I perform roughly 5-6 fat transfers to the lower eyelids weekly and can tell you that I have only seen this once.  The fact is that you may have had this vein long before the surgery, and by filling in the area, the vein was pushed to the surface and now is visible.  This, along with congestion of tissues, can cause for engorgement of the area dilating the veins.  Laser treatments are best suited to treat these issues, but please make sure the office is equipped properly and has experience.Best of luck to you.

Considering you mention that you are prone to swelling, you may want to wait another 6 months to see if things will normalize on their own. If a blue vein is still visible at that time, a YAG laser can be used to help collapse the vein and visibly get rid of it. Lasers are completely safe to use around the eyes when appropriate precautions are taken. See someone with experience treating such things as they are not that common.Andrew Campbell, M.D.

This vein was there pre-op but you probably couldn't see it.  Adding fat to the lid has pushed this vein forward so it is now more visible under the skin.  The added volume may also cause some venous congestion in the area so that the vein will be more dilated and therefore visible.  You can do some 1064 nm Yag laser treatment to close the vein.  You will need good eye protection but this laser can be used for veins in the eyelid.

After a bleph many of the small blood vessels are

electrocoagulated resulting in shrinking of those vessels.

However backflow pressure as well as  neovascularization 

may also occur.In your case it is important to establish a definitive 

diagnosis of your concern. That will determine the best treatment
