Is this procedure going to change your life?Cosmetic surgery can do many things. However, it is important to understand what it cannot do. Cosmetic surgery is not a means to an end i.e. wealth, romance, self esteem. None of these things are rooted in external beauty/appearance. Patients pursuing surgery for similar reasons are likely to be disappointed.5. Know your plastic surgeon!There are numerous practitioners of plastic surgery. However, only a select few are board certified by the American Board of Plastic Surgery (the only board recognized by the American Board of Medical Specialties). Board certification ensures completion of a rigorous training program of at least 6 years (with at least 3 years of dedicated plastic surgery training. Board certified surgeons have also completed written and oral examinations to determine competency and participate in a continuing medical education programs as a prerequisite of maintaining certification. Ultimately, certification ensures a measure of certainty when comparing surgeons. You can be assured that your board certified plastic surgeon has seen and treated it all before and is uniquely qualified to manage you pre-operatively, intra-operatively, and most importantly post-operatively.6. Know your OR!Patients should ensure that the procedure that they are undergoing is being performed in AAAASF or Medicare/JCAHO approved facility. You should get a good examination by a plastic surgeon to give you the best advice. Implants may indeed lower your nipples and make your breasts look less perky. Your implants would have to be placed quite low and you may get less upper pole fullness than in other situations.

Breast augmentation surgery is a safe pleasant surgery if performed by the right surgeon. I recommend looking for a board-certified plastic surgeon and request before and after photos. If you want to get bigger and perkier breasts and if you have overall good health there is no reason to not have this operation. You seem suitable for both over muscle and under muscle placement.
for sure it is possible to place implants which will increase the roundness in the upper part of the breast and make you look perkier. Of course, that comes with also an increase in size. Is this something that you desire? If you feel you can carry larger breasts, of course you could do implants. In my opinion, what is also of importance is the profile of the implant that you select. If for example you select a moderate profile or moderate plus profile and they are placed in the sub muscular position, one would not expect significant superior fullness or a perkier look. On the other hand a high profile or one of the higher than that profiles would be expected to make your breasts look perkier. If you feel like your breasts could be 1 to 1 1/2 cup size larger, your goals can probably be achieved.
Thank you for your photos. The effect of a push-up bra is to push the breast tissue upward so that it fills out the superior pole of your breast. You only have minimal or no ptosis of your breasts, so a breast lift would not be in your best interest. A breast implant could be placed and that would give you permanent volume in your breasts upper pole, however it would also increase your breasts size and your request was for perkier breasts, not larger breasts. Meet with a board certified plastic surgeon to have a detailed discussion so you fully understand.
Dear Vivid303916,breast implant type and sizing selection is a complicated process and requires a detailed consultation that includes a discussion of your goals, a detailed examination, and a lengthy discussion. But the decision will always start and end with the patient. When it comes to sizing, we have all of our patients find 3 photos of their desired breast size. We ask for naked breast photos because clothing can obscure the true size (push up bras). Next, we consult with patients and go over the photos and take detailed breast measurements during a physical exam. Next, we using a sizing algorithm to determine a range of implants that will work based on the exam and the measurements.Lastly, during the operation, we use silicone sizers in the operating room to see how the size actually looks in the breast. We sit the patient up in the operating room, while asleep of course, and compare the sizer result with their desired look photos. If it is too small, we start the process over with a slightly larger implant. We don't stop until we get the perfect match.Of all of my breast augmentation patients, every one of them has been happy with their size due to this detailed and meticulous process. Higher profile implants can increase perkiness to a certain extent, however your surgeon would need to let you know by how much and if it would satisfy your goals. Please note that the larger the implants, the heavier they are, and the risk of sagging can be increased. You’ll need to determine whether the risks of surgery are worth the results you’ll achieve.
Technically you could get implants but implants will need to give you some upper pole fullness so you feel the "perky" effect. However, the heavier the breasts, the saggier they get. Yours are already starting to sag. Once you go to a consultation with a board certified plastic surgeon, then you can decide between a lift or an implant.
Your breasts are wider than your chest wall and the breasts are touching in the midline. You cannot handle anymore volume but what you would benefit from is a breast lift.
