What is the likelihood of a second accutane purge?

I took accutane for about 1 year and had a pretty bad purge for about 2 months. I had moderate consistent acne and it got pretty bad during the flare up. But after the purge my skin began to clear up and at the ending of the therapy my skin was completely clear. Now I am off Isotretinoin for about 1 year and my skin got a little bit worse. Not as bad as before my first round but I consistently get pimples around my temples and cheeks. Now I am considering doing a second round of Accutane and would like to know if the chance of another purge is as high as in my first round though my acne is much much lighter. And I would consider to do a low dose of accutane to keep the likelihood of purging as small as possible. Thanks for your help.

Likelihood: The intensity of a second purge is usually less severe than the first, especially if your skin has responded well previously. The skin has already undergone major changes in oil production and follicle turnover, which reduces the risk of a dramatic flare.
Low-dose approach: Many patients do well on a lower-dose regimen during a second course, which can help maintain improvement while minimizing the chance of noticeable purging. Low-dose isotretinoin often controls acne gently without provoking the intense flare-ups sometimes seen in initial courses.
Individual variability: Everyone’s response is a bit different, so while the risk is generally lower, small breakouts in the early weeks of retreatment can still happen.

Overall, your prior response is a good indicator that a second course—especially at a lower dose—would likely be well-tolerated, with a much smaller chance of a severe purge.
