What is going on with my eye?? A few weeks after surgery my eye looked great. I was so happy with the result. it has been two months since my second ptosis surgery with a board certified oculoplastic surgeon. I expected so much more and now I have this. I saw my doctor two weeks ago and all he talked about was the 1mm difference in my eyes. What about my eyelid? he mentioned possibly a third surgery. I am hesitant to return to the same surgeon. I would really love some advice.

There are very limited resources to fix these eyelids.  Do not let this surgeon do a third surgery.  You need second opinions.  I need to know the details of your surgery but my concern is that you had a posterior surgery when you needed an anterior levator ptosis repair.  The Mullerectomy is very popular because it is very straight forward.  However, if you have a disinserted levator, the posterior surgery will not work.  Let the eyelid heal for at least 6 months before revisional surgery and find someone who knows how to fix upper eyelids.

Based on the photographs, there are a few issues going on. One is that your right eyebrow is slightly higher than your left one, which makes more of the right eyelid visible than the left one. Along those lines, the left upper eyelid has more skin on it than the right upper eyelid. That said, the nasal aspect of the right upper eyelid is ptotic compared to the left upper eyelid. The question is what type of prior surgeries were done and what approach was used - it seems like at least one approach was through the skin because one of the photographs seemed to have a faint scar. I would suggest getting a second opinion because the revision may require adjustment of previous sutures that were placed or may have to do with your anatomy, but would need a full evaluation to determine that.
