I have sun damage and malasma on my face. Went to a Dr and he did IPL (he said I needed at least 3 treatments) it made it worse. is that normal?
Answer: Clear & Brilliant combined with a chemical peel can help treat sun damaged skin as well as melasma. Fractional laser is an amazing and highly effective treatment in our office. We use the laser treatment to help with acne scars, large pores, fine wrinkles, tissue collagen loss, scars, sebaceous hyperplasia, active acne, and for brightening dark pigmentation. Although there are very strong fractional lasers such as CO2 laser, we use lighter treatments for corrective skincare and maintenance of results. Our Thulium fractional laser, clear and brilliant laser, and Ultra Thulium laser are excellent options in our clinic. We also perform CO2 fractional and erbium depending on skin tone and tolerance for downtime. Patients do have to use a Melarase cream before and after treatment to help further reduce pigmentation after laser. Melarase AM, Melarase PM, and Melapads can seriously improve surface sun damage and UV related pigmentation. Dark spots can improve with these treatments. I recommend fractional laser for incisional scars as well, especially after plastic surgery. Best, Dr. Karamanoukian Realself100 Surgeon

Answer: Melasma and IPL I do not recommend IPL for melisma patients, as heat can exacerbate the melanin and make things worse. Take a long break from lasers and the sun, and find a doctor who has a Pico laser, as this is the only laser safe for melasma patients.Best of LuckAnil Shah MD

Answer: Pigmentation and Melasma IPL treatments is used to treat pigmentation caused by sun damage or scarring. At times pigment can look worse because pigment rises to the surface depending on how damaged the skin is. The number of treatments is usually based on how much pigmentation there is. IPL can be done in combination with other treatments like peels and or hydroquinone cream. To treat melasma I would recommend Cosmelan peel. Apply sunblock SPF 30 or higher with ZINC at all times.

Answer: IPL and melasma I do not recommend treating melasma with IPL as the heat from the laser does often make the pigmentation worse instead of better. I prefer to use either chemical peels such as a Melanage peel or a Pico Genesis laser on patients with melasma. The Pico Genesis delivers energy so quickly that heat is not built up inside the skin, which is why it is safe and effective for melasma. As you probably know, it is very important that you protect your skin from sun exposure, as this makes pigmentation worse.
