52 year old female, healthy. As a side note I have blepharochalisis syndrome that affects only my right eyelid. The fat pads returned under both eyes 5 months after my lower bleph, It is more prominent under my left eye. Someone suggested Agnes RF microneedling for the fat pads, and I wanted to get an opinion on this. (My surgeon recommended laser treatment which did not help and offered to do revision surgery. Another doctor told me that due to my blepharochalisis syndrome, it's possible the fat pads can return again).

It is very difficult to make and confirm diagnosis of blepharochalisis without good history and in person evaluation. However, based on your photos it appears that you do not have any scars in lateral aspect of lower eyelids or underneath eyelashes? Lateral fat pads, which seem to be reason for your bulge, are very difficult to remove properly through conjunctival incision only; for that reason additional skin incision along lower eyelids or at least in the lateral canthus is needed to visualize and reduce this fat pads compartment.
