Hi, in 2010 I had a rhinoplasty for a hump on my nose but the plastic surgeon ignored my hanging columella. 6 months ago I had revision Rhinoplasty and the surgeon pinned up my nose tip but now I noticed my nose looks cooked and hanging columella remains as my nose looks very long. Surgeon told me to massage my nose daily to fix the crooked part, will this work? What are my options? Revision Rhinoplasty, Botox to septum, filler, or any other treatments you can recommend please. Answer: Revision Rhinoplasty Hi there! Sorry you're experiencing trouble with your revision rhinoplasty. Massaging the nose can help to a degree if there is swelling and deviation. But, for something like a hanging columella, it usually means there's a lack of support or excessive cartilage in the wrong spot. I tend to put significant cartilage grafts at the tip which are well-sutured to provide lasting support and keep the nose where I want it. On exam, I would make sure to evaluate your tip by feel and see if it's strong or weak. Botox won't likely help. Filler could be an option to change other parts of your nose to camouflage this area. It may require a revision procedure ultimately to shorten and re-support your nose. There are options and I hope you find one that's best for you.

Answer: Revision rhinoplasty While your photos don't show every view fully, your nose is long and deviated. Revision rhinoplasty is the only fix for these two things. Best to be seen in person by a well-trained rhinoplasty surgeon for a full evaluation and to develop a plan.

Answer: Nasal asymmetry and hanging columella after revision Mild asymmetry and curvature can be nicely improved with a fat graft which is much more permanent than filler and will not create scar tissue. The long nose Would require a revision but often this can be done with incisions inside the nose as an office procedure with minimal downtime. You need to discuss with your surgeon in person to give a more considered opinion.

I guess if we could fix it with massage, we would go to Thai massage parlors and have our nose massaged instead of our body. Or in that case, what is the need for surgeons, right? Joking aside, I am very angry because I find the surgeons who recommend this unfortunately very incompetent. If we go back to your subject, I want to tell you that you need a revision nose surgery again. Neither filling, botox, massage nor anything else will fix your nose. Answer: Tip Plasty It looks like you will want more of a full tip plasty than what was performed. It will look better in a couple of months when more of the swelling has resolved but it looks like you would benefit from a couple maneuvers to shorten the nose and bring the entire tip and columella more upward.
