I am 33 years old! I had fat transfer surgery about 5 years ago. Unfortunately my right breast got a big fat necrosis (it has been 1.5 years now) It is really bad and my nipple is inverted now (due to fat necrosis)! My doctor suggested to remove it by surgery and put implant but he says it might not look good! Another way is to remove all my breast and put implant to make sure my breasts look smooth and event! I am so sad because I don’t want to lose my whole breast since I don’t have cancer!

Since you are insulin resistant, you are probably prediabetic. Should you become a diabetic and go on insulin, your fat transfers will not do well. I would suggest considering implants, especially if diabetes runs in your family and you are overweight.
