I'm now in my early 30s and just this year have started getting two big folds under my eyes only when I smile. I would love to find a solution for this: 1-Can off label Botox be put in the infraorbital head of lavator labii superioris? 2-can fillers make my whole lower eyelid thicker and less prone to folding when I smile? 3-do I just need to accept that I'm getting old and this is how it is? P.S.Just last year my tear troughs were filled (perhaps too much?) Can that be the cause or is it just my age?

At your age, botox or dysport would be the way to go to relax the muscle. Another approach, usually employed for older patients with a "witch chin" would be a submental (under the chin) incision and partial removal or tightening of the muscle (which I would not recommend for you).
