Mild Tuberous/Constricted breasts: Nipples look normal/breast rounded out when cold, coney and puffy nipples when warm

My PS has decided to go inframammary with round high profile silicone, and he said he will be "scoring the internal tissues to round out the breast for the implant" but will that make my nipples appear less herniated like they do when they are cold? I love the appearance of my breasts when they are cold, was hoping that by releasing the internal tissues which he said he is doing, they will look normal? Or, will my nipples still be puffy looking because he is going the inframammary approach?
