Buccal fat excision can be done comfortably, awake, under local anesthesia in under an hour. The results are long lasting and the recover is quick. I recommend you to schedule a consultation with a board certified plastic surgeon for proper evaluation.

Sure - that can be done at the same time.  There's not really a problem with doing those together.

There is no biologic merit as to why buccal fat pad removal and cheek dimple creation can not be done at the same time. While there are in anatomic proximity neither procedure directly influences the other. While it is tempting to think that buccal fat removal helps the persistence of the cheek dimple, it does not but neither does it prevent its persistence either. The cheek dimple pathway is in front of the buccal fat pad site in the way it is surgically performed.

Both Dimpleplasty and Buccal Fat Pad removal can be performed at the same time. Actually it may make more sense in your case to do this simultaneously. When I see patients from Europe or Asia who fly into Minneapolis we always try to combine the procedures to minimize recovery and avoid unnecessary travel. 
The Dimpleplasty is performed by creating a soft, natural connection between the skin and the buccinator muscle. The buccal fat pad is behind the muscle and should not affect your results when performed by a highly skilled plastic surgeon with fellowship in cosmetic surgery.
