I am of Asian decent and 53-years old. The facial rejuvenation procedures I have had are fillers for bags and Botox for a brow lift to address heavy eyelids that make me look tired. I have lost the definition of my jaw and face is getting rounder and square at the lower half. I also have a fat pad under my chin now since menopause. I think it’s time for a facelift and maybe a heavy duty peel for my dull skin. Opinions?
Answer: Be careful what you ask for. You do not need a facelift. However, you will get what you ask for. You have upper eyelid ptosis. You would benefit from a carefully performed upper eyelid ptosis surgery with a double fold surgery. This will actually allow your eyebrows to relax down. Currently you hike up your eyebrows to compensate for the upper eyelid ptosis. The tension at the eyebrow does not help you look youthful. A TCA peel is reasonable but should be kept light. There is a risk of hyper or hypo pigmentation. Finally, rather than a facelift, I think you might benefit more from a localized lower face and neck liposuction.

Answer: Is it time for a facelift Hi, Although you are 53, you barely have excess skin, although the neck is not visible in photos. However, I think at this point, yes, you would benefit from an upper blepharoplasty, some fat grafting to face, skin peel or laser, and if there are any bands on the neck, a necklift.

Answer: Facelift I can see the facelift and eyelids for sure. The peel is not a great idea. You really don't need it and with darker skin you run the risk of pigment problems.

Answer: Blepharoplasty and Facelift It will certainly address your concerns of droopy eyes, lack of a jawline and fat under the chin. Upper eyelid blepharoplasty The excess skin in upper eyelid can be easily corrected with an upper eyelid blepharoplasty. In this procedure the excess skin is marked using a gentle pinch to assess the amount of skin that needs to be removed. The incision is placed in the natural crease above the eyelid to camouflage it. The procedure is quick and has very little down time. It can be done under local anesthesia. Lower Eyelid Blepharoplasty The lower eyelids look baggy mostly because of excess fat. This can be removed either by making an incision under the lashes called sub-ciliary approach or from inside the eyelid called Transconjunctival approach. The amount of skin removed has to be minimal as it can pull the lower lid downwards and make it look strange and change the shape of the eye. All surgeons have their favorite approaches. I like Transconjunctival approach in most cases. Lower facelift and necklift It is important to understand that the incisions used for achieving a good result in a necklift also give a nice lift to the lower face and jowls. The purpose of a necklift is to pull the extra skin side ways behind the ears and remove it. It is also sometimes combined with Neck Liposuction and Platysmaplasty. The incision starts in the hair tuft in front of the ear and then curves in front of the ear hidden in natural curves of the ear, then next to the earlobe it goes behind the ear in the hidden groove and finally is taken over to the hairline of the neck. The skin is then lifted to expose the underlying platysma muscle. It is then pulled back and tethered to the bone behind the ear. At this point the jowl area is also pulled back with stitches under the skin in deeper tissues. These maneuvers lead to excess skin on the sides which is then trimmed and stitched back in place.
