Is Scarring That Extends From the Infra-gluteal Crease worth it?

Since implants are not an option, and fat transfer may not provide the desired result, It seems like a lower butt tuck is my only option. I'd like to know if my expectations from this procedure are realistic (based on the attached illustration). The only doc in my area that does this, leaves a scar that extends from the infra-gluteal creases to the mid of the thighs. Is this a good trade off, given my condition?
