I have always known my breasts are some what saggier than most other girls I knew. But I recently saw a site which describes breast ptosis in 3 degrees from mild-moderate-extreme. Is it true that if the nipple is just 3cm under the fold then they have extreme ptosis? If so, where does that leave breasts where the nipple is 12inches below the fold? How rare is ptosis as extreme as mine, and will they get worse? Also, are my areolas a lot larger than average, and will they get bigger?

From observing the pictures , you appear to have grade 3 ptosis (severe), with very large areolas , which are positioned too medially (ideally should be position along the golden triangle) . Your breasts appear large - D or DD cup size. Your condition is not rare, especially after having children. The solution is breasts reduction with key hole technique. That will give you smaller and perkier breasts with smaller and more attractive areolas. From the partial view of your abdomen, it appears that you have significant skin and muscles laxity. So, logically speaking, I would recommend to you to have first tummy tuck surgery , which will give you, not only flat stomach with less stretch marks, but also, will make you lose weight due to the abdominal muscles tightening. This is because, the increase pressure on your stomach will reduce the amount of food that you eat and the sense of hunger. In summary, you will look more attractive with better posture and be healthier. Due to the general body weight loss in 6-8 weeks, you might need only breasts lift as oppose to breasts reduction. If you would like perky breasts without droop, then you would need a breast lift with an inverted "T" scar. When your breasts are lifted, they will become smaller and therefore, you may want to also have volume replaced through an implant. Your areola can be made smaller with the breast lift surgery

By definition, if the central point of your nipple areola complex (NAC) is more than 3 cm below your breast crease (IMF), you have Grade 3 ptosis. This is considered severe ptosis. I recommend a lift (also known as mastopexy) to my patients with similar breasts to yours. This is a powerful operation where the NAC is lifted to the level of the IMF.
