To what degree of ptosis are my breasts? (photos)

You have grade 3 ptosis- your nipples are at them bottom of your breasts, pointing down and below the creases. The grades are merely for classification, it does not mean anything is wrong. Treatment is similar for all stages and involves a lift with or without implants in most cases. An exam and consultation with a plastic surgeon is recommended to discuss your options and expectations.

Dear Sarah ,   Thanks for your pictures. From observing the pictures , you appear to have grade 3 ptosis (severe), with very large areolas , which are positioned too medially (ideally should be position along the golden triangle) . Your breasts appear large - D or DD cup size.  Your condition is not rare, especially after having children. The solution is breasts reduction with key hole technique. That will give you smaller and perkier breasts with smaller and more attractive areolas.  From the partial view of your abdomen, it appears that you have significant skin and muscles laxity. So, logically speaking, I would recommend to you to have first tummy tuck surgery , which will give you, not only flat stomach with less stretch marks, but also, will make you lose weight due to the abdominal muscles tightening. This is because, the increase pressure on your stomach will reduce the amount of food that you eat and the sense of hunger. In summary, you will look more attractive with better posture and be healthier.   Due to the general body weight loss in 6-8 weeks, you might need only breasts lift as oppose to  breasts reduction. This procedure is simpler, less costly and has less complications.         Always, consult with experienced board certified plastic surgeons who operate in accredited surgery center , for your safety. Thank you for your question and photographs.  You appear to have grade 3 ptosis with your nipples at the lowest point of your breast mound, below your breast crease and pointing towards the ground.  This degree of ptosis is not uncommon and many patients with this elect to have a breast lift performed to narrow their areola and elevate their breast tissue higher onto their chest.  The lift also allows removal of excess skin that has developed as the breasts have sagged.  See an ASPS board certified plastic surgeon if you are interested in exploring treatment options to provide enhancement. 

Thank you very much for sharing your concerns with us.After having analyzed all the information and photos sended to us, i can realize that you have uneven and sagging breasts, with more pronounced fall on the right side than the left. Also you have very large areolas related breasts size/shape.In my practice for cases like yours where the breasts are very sagging but with a good size i perform a Breast Lift - Symmetrization Surgery, removing more tissue on the right side than the left to match both breasts.I also recommend an Areolar Reduction to get an appropiate areolar diameter, with hidden scar (a circumference line surrounding the areola).There is no need to use implants, because you have very good breasts volume, and at your young age, your breasts are mostly made up of fibro-glandular tissue, therefore they will have very good consistency to the touch after Breast Lift.Respectfully,Dr. Emmanuel Mallol Cotes.-

Thank you for the question. Your breasts clearly demonstrate third degree ptosis. By definition, if the central point of your nipple areola complex (NAC) is more than 3 cm below your breast crease (IMF), you have Grade 3 ptosis. This is considered severe ptosis. I recommend a lift (also known as mastopexy) to my patients with similar breasts to yours. This is a powerful operation where the NAC is lifted to the level of the IMF.  Be sure to consult with a board certified plastic surgeon who is comfortable performing this procedure. Based upon your photographs, your breasts demonstrate third degree ptosis, where the nipples are located at the bottom of the breasts and point downward (degrees of ptosis compare nipple position to the level of the inframammary fold).  Options are no ptosis, first degree where the nipple is at the level of the fold, second degree where the nipple is below the fold and third degree where the nipple points downward.  Pseudoptosis describes a breast where the nipple is above the fold but the breast tissue droops significantly below the fold.There is no way to predict how your breasts will age / appear over time.  Repositioning the nipples and reducing the size of the areolae can be accomplished by undergoing a breast lift (mastopexy).If you are considering breast lift surgery, I would recommend that you consult with board-certified plastic surgeons who are experienced at performing breast lifts.  The surgeons can review the procedure with you in detail, and help you to decide if it is appropriate for you.Best wishes.

You should make an appointment with a plastic surgeon for an in person consultation. It does look like grade 3 ptosis and you could be helped with a breast lift. There are a variety of techniques for these operations. Most commonly there is an incision around the areola and another between the areola and breast crease. This technique is referred to as the “lollipop” technique and avoids a long scar in the crease under the breast (older anchor technique). On occasion, especially with mastopexy, these incisions may be modified and more limited. If breast sagging is minimal, the procedure may only require the incision around the areola. It is unusual to use the “anchor” or “inverted T.” This is an older method that creates as much as twice the scar and can flatten the breast, creating less forward projection and suboptimal shape. The nipple-areolar complex is repositioned higher, the excess skin is removed, and the breast is reshaped to a pleasing contour that is more normally positioned.In some cases, the insertion of an implant, may be advisable, or your own fat can be transferred to enlarge and shape the breast. This is the “California Breast Lift” or Natural Breast Enlargement.

Thank you for the question and photos. You appear to have grade 3 ptosis and asymmetry. This can be safely corrected with a breast lift procedure using a vertical lift (lollipop) incision to lift your breasts, make your areolas smaller and correct the asymmetry. Breast ptosis is very common and you are not the rare case.  A full lift may be a good option for you to correct this issue.

     You appear to have third degree ptosis.  The measurements are less important than what your final objective is.  If you would like perky breasts without droop, then you would need a breast lift with an inverted "T" scar.  When your breasts are lifted, they will become smaller and therefore, you may want to also have volume replaced through an implant.  Your areola can be made smaller with the breast lift surgery
