Can non surgical rhinoplasty lift my tip and straighten my nose and help make my nostrils look smaller? (photos)

Hello and thank you for your question. Unfortunately no, fillers would not be able to take the place of surgery, especially in your situation. It sounds like your biggest concern is your crooked nose, asymmetric nostrils, drooping/deviating tip with smiling, and overall nasal imbalance. These are due to the underlying anatomy present- a combination of your nasal bones and cartilage which makes the structural framework of your nose, as well as a muscle (the depressor septi nasi muscle) which contracts when you smile and causes the animation deformity. Also, you have a high radix (portion between your eyes). The combination of all these things is not amenable to non-surgical treatment, and would be most appropriately treated with surgery. Addressing the shape of the nose and correcting nasal imbalance (especially at the tip) are actually some of the most common reasons people seek out and undergo a rhinoplasty procedure. Also, the animation deformity (the muscle) can be easily addressed during the procedure.

In addition to addressing your aesthetic concerns, your septal cartilage (or an off-the-shelf alternative) would be harvested (removed) and turned into multiple cartilage grafts, which would then be secured into different areas of your nose for structural support, including in the columella (middle bottom portion of your nose between your nostrils) to really reinforce the tip and prevent long term alterations/distortions to nose shape.

The open approach is very effective in facilitating complex, precise maneuvers to refine the nasal shape and size to a particular aesthetic and look. Rhinoplasty is arguably the most difficult procedure to perform in all of plastic surgery. I would only recommend going to see a surgeon who feels very comfortable with both primary and revision rhinoplasty procedures, and who has significant training and experience in these types of surgeries. This is a surgery of millimeters, so if the surgeon you're going to only does 1-2 per month, you are not going to get the best result possible.

Of course it is always difficult to provide tentative surgical recommendations, as there is only so much information to be gained from the photos and description provided.

Based on your photos, you appear to have relatively thin nasal skin with a crooked nasal appearance that is likely due to a deviated nasal septum, as seen on the base view of your nose. This is associated with a tension lip deformity, excess nostril show, a wide tip, and a sizable dorsal hump. The non-surgical rhinoplasty can lift the tip and make the nose appear straighter. It will not decrease the appears of your nostrils. The difference in appearance of the tip when smiling is related to a tiny muscle called the depressor nasi septi. If this part is especially troublesome to you, a little botox in this muscle can help.
