Hard lumps and painful swelling after receiving temple filler over a year ago, is surgery the only option to remove them?

It sounds like you have a biofilm which responds to Biaxin 500 mg twice a day, but patients must take it for 6 straight weeks, if it does not completely disappear, then another 6 week course is given. Discuss this with your treating physician. We sometimes use hyaluronidase as well to help dissolve the product that the biofilm is attracted to. Good luck and be well.

First of all, it would be most helpful if you can list all the products used and also the Hylase doses used. Did your injector use an ultrasound to guide the Hylase?
If the product is purely hyaluronic acid, then with proper dose and placement of hyaluronidase, it should improve significantly.
This kind of delayed reactions usually do not settle well with antibiotics. Short term treatments are best carried out with cautious use of hyaluronidase if the material is hyaluronic acid only.
The discussion of granulomatous lesions and biofilm still remains controversial.
