I had a breast lift and implant a little under a year ago. Tonight I noticed I have a smaller hole at the bottom of my nipple with a white wire that I can touch and move around inside if it and starting to come out. What could that be?!

If you are happy with your size, then get a lift. If you want more fullness, add implants. Please do not go too large with implants or you will sag again from the weight.
Don't choose the crescent lift since it can change the shape of your areola. You might do well with the doughnut lift, but measurements are needed to be sure. I would suggest not going with such a large and heavy implant, but something that fits your frame and tissue.
For women who do not have much sagging, radiofrequency treatment like ThermiTight can be done to tighten loose skin and help lift the nipple with tiny incisions.
